尿道板重建卷管尿道成形术在复杂尿道下裂矫治中的应用

Plate reconstruction and tubularization urethroplasty in the repair of complicated hypospadias

目的 探讨尿道板重建卷管尿道成形术（PRTU）在复杂尿道下裂矫治中的应用.方法 回顾性分析2010年9月至2014年7月应用PRTU方法矫治145例尿道下裂的病例资料.本组年龄10个月到37岁2个月,平均4岁4个月.其中初治病例135例（阴茎近段型1例,阴囊型76例,会阴型56例,无尿道下裂阴茎弯曲畸形Ⅳ型2例）,再手术10例（9例一期手术后,1例术后成形尿道裂开并残留阴茎弯曲）.PRTU以单纯或联合岛状包皮瓣方式应用于手术矫治,尿道重建一期或分期完成.结果 本组一期矫治84例,其中单纯PRTU一期矫治65例,PRTU联合岛状包皮瓣19例；分期矫治61例,其中单纯PRTU分期56例,PRTU联合岛状包皮瓣分期1例,PRTU联合颊粘瓣分期4例.矫正下曲后尿道缺损长度2.0～11.5 cm,平均4.6cm.术后随访3个月到3年11个月,平均1年7个月.行PRTU一期矫治患者的总并发症发生率为16.9％（11/65）,其中尿瘘4.6％ （3/65）,尿道狭窄3.1％ （2/65）,阴茎头裂开9.2％（6/65）,尿道憩室1.5％（1/65）.行PRTU联合岛状包皮瓣一期手术患者的总并发症发生率为10.5％（2/19）,其中尿瘘5.0％（1/19）,尿道憩室5.0％（1/19）.行PRTU分期一期手术患者的总并发症发生率为4.9％（3/61）,其中尿瘘4.9％（3/61）；36例完成二期手术患者的并发症发生率为16.7％（6/36）,其中尿瘘8.3％ （3/36）,尿道狭窄2.8％（1/36）,阴茎头裂开5.6％ （2/36）,尿道憩室2.8％（1/36）.结论 PRTU可依据一定指征用于矫治局部组织条件较差的重型尿道下裂和合并明显尿道短缩性阴茎下曲的再手术患者,可灵活地单用或联用,也可一期或分期矫治.

Objective To evaluate the efficacy of plate reconstruction and tubularization urethroplasty （PRTU） in the repair of complicated hypospadias.Methods From September 2010 to July 2014,145 patients with hypospadias were repaired with PRTU technique.The average age was 52 （10-86） months.Among them,primary cases included proximal shaft （n =1）,scrotal （n =76）,perineal （n =56） and type Ⅳ chordee without hypospadias （n =2）.And secondary cases included post-stage-Ⅰ （n =9） and urethral dehiscence （n =1）.PRTU was performed alone,or plus island tube or buccal mucosa,as single-stage or staged repair.Results A total of 84 patients were repaired in single-stage including PRTU alone （n =65） and plus island tube （n =19）.Staged repair was performed with PRTU alone （n =56）,plus island tube （n =1） and plus buccal mucosa （n =4）.During a mean follow-up period of 19 （3-47） months,the complications during single-stage PRTU repair occurred at a rate of 16.9% （11/65）,including fistula 4.6% （3/65）,stricture 3.1% （2/65）,glans dehiscence 9.2%（6/65） and urethral diverticulum 1.5% （1/65） ; single-stage plus flap 10.5% （2/19）[fistula 5.0%（1/19） and urethral diverticulum 5.0%（1/19）]; stage Ⅰ 4.9%（3/61）（fistula 3/ 61） and in 6/36 patients stage Ⅱ 16.7% [fistula 8.3% （3/36）,stricture 2.8% （1/36）,glans dehiscence 5.6%（2/36） and urehral diverticulum 2.8%（1/36）].Conclusions PRTU may be indicated in the repair of primary or redo hypospadias with poor quality of local tissue.It can be used alone or plus island tube or free graft during single-stage or staged repairs.
