疱疹病毒性皮肤病在反射式共聚焦显微镜下的图像特征 被引量:4

摘要 目的:明确单纯疱疹、水痘、带状疱疹在体反射式共聚焦显微镜(RCM)下的图像特征。方法:对临床初步诊断为单纯疱疹、水痘的患者进行RCM多层扫描,观察镜下特征;取完整水疱进行组织病理检查。结果:RCM与病理诊断相符;疱疹病毒水疱RCM特征表现为表皮内界限清晰地、低折光的液性水疱形成;疱内可见较高折光的较大圆盘状细胞(含病毒包涵体的角质形成细胞)、单核及分叶核炎细胞。结论:疱疹病毒性皮肤病在RCM图像下具有较特异的特点,可以为疱疹病毒感染皮肤病诊断提供线索。 Objective:To determine the image characteristics of dermopathic herpesvirus disease under reflectance confocal microscopy (RCM).Methods:The image characteristics of forty patients who were initially diagnosed as herpes simplex (25 patients) and varicella (15 patients) were observed under RCM.The histopathologic examination of blister was taken as a reference.Results:The results of RCM were consistent with histopathologic examination.The image of dermopathic herpesvirus disease under RCM were characterized by low refractive blisters in clear boundaries within the epidermis,large high refractive disc cells (keratinocytes containing viral inclusions),and monocyte and segmented granulocyte in blisters.Conclusion:The image of dermopathic herpesvirus disease under RCM is specific,which can provide the clues in diagnosis.
