贵港市HIV/AIDS病人生存时间及影响因素分析

Survival time of HIV/AIDS victims and its influencing factors in Guigang City

目的了解广西贵港市艾滋病病毒感染者及病人的生存时间分布特征及其影响因素。方法采用回顾性研究,采用艾滋病综合防治数据信息系统中截止2013年12月31日,按所有的现住址为本地艾滋病病毒感染者及病人为研究对象,收集死亡报告病例相关信息,采用Cox比例风险回归模型分析艾滋病病毒感染者及病人生存时间的影响因素。结果死亡的2 855例艾滋病病毒感染者及病人中,2000-2007、2008、2009、2010、2011、2012、2013年分别死亡人数274人、274人、385人、407人、567人、648人、300人。平均死亡年龄为（56.71±17.10）（0.17~96岁）,研究对象生存时间的中位数是9个月,接受和未接受过抗病毒治疗者的中位生存时间分别是18个月和7个月,二者差异有统计学意义（P=0.000）。多因素Cox回归分析结果显示从确证到抗病毒治疗开始时间、传播途径、是否抗病毒治疗是影响病人生存时间的密切因素。结论 2000-2013年,贵港市HIV/AIDS病人生存时间的中位数为9个月,从确证到抗病毒治疗开始时间、传播途径、是否抗病毒治疗是病人生存时间的影响因素。

Objective To analyze the distribution characteristics of the survival time of HIV/AIDS victims in Guigang City, Guangxi, and to explore its influencing factors. Methods The retrospective study recruited HIV infected persons and patients who were registered into AIDS prevention and control information system as of December 31, 2013 and who were marked as local residents in accordance with their self- claimed addresses. Information of the death report cases were collected. Factors affecting the survival time of the HIV infected persons and patients were analyze with Cox proportional hazard regression model. Results Among Lhe 2,855 death cases, 274 , 274 , 385 , 407 , 567 , 648 and 300 died in 2000--2007,2008,2009,2010,2011, 2012 and 2013 respectively. The death age ranged from 0.17 to 96 years, with an average of 56.71 ± 17.10 years. The median survival time of all the death cases was 9 months, and that of those with anti - virus therapy and of those without were 18 months and 7 months respectively, showing significant difference （ P = 0. 000）. Cox proportional hazard regression analysis indicated that the lag between confirmed diagnosis and onset of antiviral therapy, transmitting route and antiviral therapy were the main influ- encing factors of the survival time. Conclusions The median survival time of HIV/AIDS victims in Guigang City in 2000 - 2013 is 9 months. The lag between confirmed diagnosis and onset of antiviral therapy, transmitting route and antiviral therapy are the main influencing factors.
