血浆D-二聚体及血气分析与慢性阻塞性肺疾病相关性探讨

The correlational study between plasma D-dimer,blood gas analysis and chronic obstructive pulmonary disease

目的探讨血浆D-二聚体的检测和血气分析与慢性阻塞性肺疾病的相关性。方法确诊的59例慢性阻塞性肺疾病患者作为观察对象(观察组),另选59名健康人作为对照组。观察组又分为病患在发作时(发作组)和病情稳定后(稳定组)两个亚组,检测两组患者的D-二聚体并对患者血气进行分析,同时检测稳定组与对照组的D-二聚体以及血气值。对比两组的测定结果。结果发作组与稳定组患者D-二聚体均显著高于对照组(P<0.05);发作组D-二聚体、二氧化碳分压显著高于稳定组患者(P<0.05);发作组的pH值、氧分压均低于稳定组患者,差异均有统计学意义(P<0.05)。结论血浆内的D-二聚体检测值和动脉血气的各项分析指标与慢性阻塞性肺疾病显著相关,pH值、二氧化碳分压以及氧分压数值随着D-二聚体的改变而变化。可作为慢性阻塞性肺疾病的辅助诊断,也为临床病情程度的判断、治疗和预后提供了有价值的依据。

Objective To investigate the correlation between plasma D-dimer detection, analysis of blood gas and chronic obstructive pulmonary disease. Methods From 2012 May to 2013 May, 59 patients suffered chronic obstructive pulmonary disease was taken as the observation group, another 59 healthy people was taken as control group. The observation group was divided into two subgroup, attack group and stable condition group, according to whether the patient was in stable condition. We detected plasma D dimer and analysed blood gas of the attack group and stable condition group, at the same time, detected and analysed the observation group and the control group, comparing the tested result of two groups. Results The detection value of plasma D dimer of attack group and stable condition group was significantly higher than that of the control group （P〈0.05）. The detection value of plasma D dimer and CO2 partial pressure of attack group was significantly higher than that of control group （P〈0.05）. The pH value, oxygen partial pressure of attack group was significantly lower than that of stable condition group（all P〈0.05）. Conclusion The detection value of plasma D dimer and arterial blood gas index have significant correlation with chronic obstructive pulmonary disease. The PH value, CO2 partial pressure and oxygen partial pressure were changed by the detection value of plasma D dimer, which can be a auxiliary diagnosis of chronic obstructive pulmonary disease, providding valuable basis for prognostication, treatment and prognosis of clinical condition.
