DWI Quantitative Evaluation of Minimally-Invasive Treatment for Non-Compressive Radiculitis: An Experimental Study in Chinese Miniature Pigs

摘要 目的探讨扩散加权成像(DWI)技术对腰椎非压迫性神经根炎不同微创治疗术后变化定量评价的价值。方法中国小型猪12只CT导向下非压迫性神经根炎造模成功14天后,根据治疗方法不同随机分为4组。分别于实验猪的L2~3、L3~4及L4~5双侧神经根旁注入相应物质进行治疗。每组分别于术前、术后3天、7天及15天对实验猪进行MRI扫描,扫描完成后处死1只实验动物,取相应双侧神经根行免疫组织化学检查。分析各组于术前、术后3天、7天、15天神经根表观扩散系数(ADC)值及神经根CD8阳性T淋巴细胞计数评分的变化,以及两者之间的相关性。结果 A、B、C、D组治疗前、治疗后3天、7天、15天神经根平均ADC值及神经根淋巴细胞计数评分均呈下降趋势(P〈0.05);A、B、C、D组神经根ADC值与神经根淋巴细胞计数评分均呈正相关。结论不同治疗方法均能使神经根炎得到较好地改善,DWI技术能对腰椎非压迫性神经根炎微创治疗术后变化进行定量评价。 Objective To discuss the application of diffusion-weighted imaging in quantitatively evaluating various minimally-invasive techniques for the treatment of non-compressive radiculitis.Methods Non-compressive radiculitis model was established in 12 Chinese miniature pigs with CT-guided technique.Fourteen days later,the pigs were randomly divided into 4 groups according to the implemented treatment methods.The corresponding specific therapeutic materials were injected into the bilateral nerve root side of L2- 3,L3- 4and L4- 5.MRI scanning was performed in all the experimental pigs before the injection as well as 3,7 and 15 days after the injection.Every one pig from the four groups was sacrificed each time after the completion of MRI examination.The specimens of the corresponding bilateral nerve root were collected for immunohistochemical examination.ADC values and nerve root CD8 positive T lymphocyte count score were determined in all pigs before the injection as well as 3,7 and 15 days after the injection,and the correlation between ADC value and CD8 positive T lymphocyte count score was evaluated.Results ADC value of nerve root as well as CD8 positive T lymphocyte count in all the four groups were decreased before the treatment as well as at 3,7 and 15 days after the treatment( P 〈 0.05).A parallel correlation existed between ADC values and lymphocyte count scores.Conclusion All treatment methods can well improve the nerve root inflammation,and diffusion-weighted imaging can make quantitative evaluation of noncompressive radiculitis after minimally-invasive treatment in experimental pigs.
