阑尾孔周围点片状炎性改变在溃疡性结肠炎内镜诊断中的意义

The Significance of the Inflammation in Peri-appendiceal Orifice in the Diagnosis of Ulcerative Colitis

目的研究内镜下阑尾孔周围炎性改变在溃疡性结肠炎诊断中的意义。方法收集四川大学华西医院2014年本院内镜下出现阑尾孔周围炎性病变的病例共28例,排除阑尾孔周围溃疡、息肉样增生的病例。结合临床表现、组织病理及随访结果分析阑尾孔周围点片状炎性病变与溃疡性结肠炎的关系。结果 28例阑尾孔周围炎性病变的病例中,8例诊断为溃疡性结肠炎,1例诊断为疑似溃疡性结肠炎。结论阑尾孔周围点片状炎性改变是溃疡性结肠炎的一种跳跃性病变,临床工作中若发现阑尾孔周围炎性改变需警惕溃疡性结肠炎的可能。

Objective To analyze the relationship between the inflammation in peri-appendiceal orifice and the diagnosis of ulcerative colitis;to investigate the significance of the inflammation in peri-appendiceal orifice in diagnosis of the ulcerative coli-tis. Methods The endocopic changes（inflammation in peri-appendiceal）of 28 patients in our hospital in 2014 were analyzed. Pa-tients with ulcer,polyps hyperplasia were excluded. We integrate the clinic mesfest,follow-up results with pathological biopsy to an-alyze the relationship between inflammation in peri-appendiceal orifice and ulcerative colitis. Results of the total 28 patients with characteristic change under the endoscope,8 patients were diagnosed as ulcerative colitis,1 was diagnosed as suspected ulcerative colitis. Conclusion Inflammation in peri-appendiceal is a skip lesion of ulcerative colitis and in the clinic work,we need to be a-lert to the possibility of ulcerative colitis when we find inflammation in peri-appendiceal.
