幽门螺杆菌感染性胃癌组织中Legumain、MMP-9的表达及其临床意义

The express of Legumain,MMP9 in Helicobacter pylori infective gastric carcinoma tissue and its clinical significance

目的探讨幽门螺杆菌(H.pylori)感染性胃癌组织中半胱氨酸蛋白酶(Legumain)、基质金属蛋白酶-9(MMP-9)的表达情况及其临床意义。方法选取手术切除胃癌组织标本109例,采用Warthin-Starry银染色法检测H.pylori感染情况,根据其H.pylori感染情况分为H.pylori阳性组59例和H.pylori阴性组50例,同时选取癌旁正常胃组织标本55例作为对照组,采用免疫组化方法检测各组Legumain、MMP-9表达情况。结果胃癌组织Legumain阳性率、MMP9阳性率均明显高于对照组(P均<0.05)。H.pylori阳性组Legumain、MMP-9表达均与患者性别、组织分化程度、临床分期和淋巴结转移相关;其中男性、组织学分化程度越低、临床分期越高及合并淋巴结转移患者Legumain阳性表达率、MMP-9阳性表达率均明显高于H.pylori阴性组(P均<0.05);Legumain、MMP-9阳性表达率越高;经Pearson相关性分析发现:胃癌组织Legumain与MMP-9表达呈正相关(r=0.46,P<0.05)。结论胃癌组织Legumain、MMP-9呈过度表达,其中H.pylori感染性胃癌组织Legumain、MMP-9阳性表达更显著,且两者表达与胃癌病情发生与发展密切相关,H.pylori感染可作为胃癌发生和发展的独立危险因素之一。

Objective It is to observe the express of Legumain,MMP9 of Helicobacter pylori infective gastric cancer,and analyze the clinical significance. Methods 109 cases of gastric cancer tissue were collected to detect Helicobacter pylori（ H.pylori） infection by Warthin-Starry silver staining method,and were divided into H. pylori positive group（ n = 59） and H. pylori positive group（ n = 50） according to H. pylori infection. 55 cases of normal gastric tissue were selected as control group at the same time. The expressions of Legumain,MMP9 were detected by immunohistochemistry for all cases. Results The positive expression rate of Legumain and MMP- 9 in gastric cancer tissue were significantly higher than that of control group（ all P 〈0. 05）,while the positive expression rate of Legumain and MMP- 9 in H. pylori positive group were significantly higher than those in H. pylori negative group（ all P 〈0. 05）. The expression of Legumain and MMP- 9 were associated with gender,histological differentiation,clinical stage and lymph node metastasis,which of Male,histological differentiation degree of lower,clinical stage of higher and lymph node metastasis with higher positive expression rate of Legumain and MMP- 9. Pearson correlation analysis showed that the Legumain express in gastric cancer was positively correlated with MMP- 9 express in gastric cancer（ r = 0. 46,P 〈0. 05）. Conclusion Legumain,MMP- 9 are highly express in gastric cancer,and both positive express are higher in H. pylori infective gastric cancer,and are closely related to the occurrence and development of gastric cancer. H. pylori infection can be used as one of the independent risk factors for the occurrence and development of gastric cancer.
