双向等压引流根治术治疗高位肛瘘的临床研究

Clinical research on bidirectional isallobaric drainage radical resection for high anal fistula

目的:探讨双向等压引流根治术在治疗高位肛瘘的临床疗效。方法:将60例高位肛瘘患者随机分为治疗组和对照组,治疗组采用双向等压引流根治术,对照组采用低位切开高位挂线术,观察两组创面平均愈合时间、治愈率、复发率、术后疼痛、尿潴留、肛门功能评分情况,并对以上指标进行量化评分。结果:治疗组创面愈合速度显著快于对照组;治疗组治愈率及复发率比较优于对照组;两组患者总有效率比较无差异;两组在术后疼痛、尿潴留、肛门功能评分比较具有显著性差异,治疗组优于对照组。结论:双向等压引流根治术能充分保护肛门功能,提高治愈率,减少并发症,操作安全可行,是治疗高位肛瘘的有效方法。

Objective： To explore bidirectional isallobaric drainage radical resection in the treatment of clinical efficacy of high anal fistula; Methods： 60 patients were divided into two groups with randomized method.30 cases were treated with bidirectional isallobaric drainage radical resection and 30 cases were treated with the low incision and high seton therapy.Detailed observations the change of the average wound healing time,the cure rate,the recurrence rate,postoperative pain,uroscesis and the score of anal function,and indicators to quantify the above score.Results： The speed of the treatment group healed significantly faster than the control group; The treatment group is better than the control group in the cure rate and the recurrence rate; No difference in the curative rate was found between the two groups; The treatment group is better than the control group in the aspects of postoperative pain,uroscesis and the score of anal function.Conclusion： Bidirectional isallobaric drainage radical resection is a safe and feasible operation which can fully protect the anal function,improve the cure rate and reduce complications,so it is an effective method for the treatment of high anal fistula.
