PET-CT定位下调强放疗同步化疗对局部晚期非小细胞肺癌患者疗效

摘要 目的探讨PET-CT定位下调强放疗同步化疗治疗局部晚期非小细胞肺癌的效果。方法选取入院治疗的局部晚期非小细胞肺癌患者60例作为研究对象,随机将患者分为对照组和观察组,对照组采用常规放疗同步化疗,观察在PET-CT定位下调强放疗同步放疗,观察两组近期疗效和毒副反应发生率和GTV计划参数。结果观察组近期治疗有效率为86.67%显著高于对照组的60.0%,差异具有统计学意义(P<0.05);观察组皮肤反应、发射性肺炎的发生率为13.33%、16.67%均显著低于对照组,差异具有统计学意义(P<0.05);观察组PETCT图像GTV平均体积为(105.32±5.96)cm3显著低于对照组的(142.62±6.78)cm3,差异具有统计学意义(P<0.05)。结论 PETCT定位下调强放疗同步化疗能提高靶区分布合理性,可提高靶区域照射剂量,提高近期疗效。 Objective To explore the effects of intensity modulated radiotherapy concurrent with chemotherapy under the positioning of PET-CT on patients with locally advanced non small cell lung cancer.Methods60 patients admitted into the hospital for treatment of locally advanced non small cell lung cancer were selected as the research object and randomly divided into the control group and the observation group. The control group was treated with conventional radiotherapy concurrent with chemotherapy while the observation group with intensity modulated radiotherapy concurrent with chemotherapy under the positioning of PET-CT. The recently curative effects, the incidence of side effects and GTV planning parameters of the two groups were observed.Results The recently curative effective rate of the observation group was 86.67% which was higher than 60% of the control group and the difference was statistically signiifcant (P 〈 0.05), the incidence rates of skin reaction and radiation induced pneumonia of the observation group were 13.33% and 16.67% which were signiifcantly lower than those of the control group and the difference was statistically significant (P 〈 0.05), the average volume of PET-CT image GTV of the observation group was (105.32±5.96) cm^3 which was signiifcantly lower than (142.62±6.78) cm^3 of the control group and the difference was statistically signiifcant (P〈0.05).ConclusionIntensity modulated radiotherapy concurrent with chemotherapy under the positioning of PET-CT can improve the rationality of target distribution and the irradiation dose in target area, improving the curative effects.
