中西医结合治疗高尿酸血症和痛风效果观察

Effect Observation of Combination of Traditional Chinese and Western Medicine in Treatment of Patients with Hyperuricemia and Gout

目的:探讨中西医结合治疗高尿酸血症和痛风患者的效果。方法:选取2012年2月—2014年2月医院收治的高尿酸血症和痛风患者150例,按随机数字表法分为观察组和对照组,每组75例。对照组给予常规西医疗法,观察组在此基础上加用自拟四妙汤,每日1剂,疗程2个月。比较两组血尿酸、血沉、CRP、肾功能指标变化及不良反应,并评价临床疗效。结果:观察组总有效率为96.0%(72/75),显著高于对照组的84.0%(63/75),差异有统计学意义(χ2=4.26,P<0.05)。两组治疗后UA、ESR、CRP均较治疗前显著降低(t=5.45、5.20、4.78、4.56、4.35、4.39,P<0.05),两组比较观察组较对照组降低明显(t=4.11、4.02、4.55,P<0.05)。两组治疗后血肌酐、尿素氮均较治疗前显著降低(t=5.45、5.20、4.78、4.56,P<0.05),两组比较观察组较对照组降低明显(t=4.11、4.02,P<0.05)。观察组不良反应发生率为2.7%(2/75),对照组为12.0%(9/75),差异有统计学意义(χ2=4.56,P<0.05)。结论:相比西医常规疗法,中西医结合治疗高尿酸血症和痛风患者能显著提高疗效,能进一步降低血尿酸,消除炎症,改善肾功能,降低不良反应,值得临床推广应用。

Objective： To investigate the efficacy of combination traditional Chinese medicine and Western medicine therapy in patients with hyperuricemia and gout. Methods： 150 cases of patients with hyperuricemia and gout from 2012 February to 2014 February treated in our hospital were randomly divided into the observation group and control group with75 cases in each group. The control group was given conventional western medicine therapy,the observation group based on this use self-made SIMIAO Decoction,one dose daily,the treatment course was 2 months. The blood uric acid,erythrocyte sedimentation rate,CRP,change of renal function and adverse reactions of two groups were compared,and evaluation of clinical efficacy. Results： The total effective rate of observation group was 96%（ 72 /75）,significantly higher than that in control group 84%（ 63 /75）,the difference was statistically significant（ χ^2 = 4. 26,P〈0. 05）. The UA,ESR and CRP of two groups were significantly decreased after treatment（ t = 5. 45,5. 20,4. 78,4. 56,4. 35,4. 39,P〈0. 05）,which in the observation group decreased significantly than that in the control group（ t = 4. 11,4. 02,4. 55,P〈0. 05）. The blood creatinine,urea nitrogen of two groups decreased significantly after treatment（ t = 5. 45,5. 20,4. 78,4. 56,P〈0. 05）,which in observation group decreased significantly that in the control group（ t = 4. 11,4. 02,P〈0. 05）. The incidence of adverse reaction in the observation group was 2. 7%（ 2 /75）,in the control group was 12%（ 9 /75）,the difference was statistically significant（ χ^2= 4. 56,P〈0. 05）. Conclusion： Compared with routine western medicine therapy patients with hyperuricemia and gout,combination traditional Chinese medicine and Western medicine therapy can significantly improve the efficacy,can further reduce blood uric acid,eliminate inflammation,improve kidney function,reduce adverse reactions,is worthy of clinical application.
