直肠癌术前固定野动态调强与容积旋转调强的剂量学比较 被引量:6

摘要 目的比较固定野动态调强(dIMRT)与容积旋转调强(RapidArc)两种技术在直肠癌术前放疗中的剂量学差异。方法选取10例直肠癌术前患者分别设计5F-0°-d IMRT、5F-180°-dIMRT和RapidArc三组计划,计划靶区(PTV)处方剂量为50Gy/25次,2Gy/次。在95%体积的PTV达到处方剂量前提下,比较两种技术的剂量体积直方图(DVH)、靶区和危及器官剂量、靶区剂量适形度(CI)、剂量分布均匀性(HI)、正常组织低剂量体积(B-P)、机器跳数(MU)以及治疗时间(TT)。结果 RapidArc计划的CI最好(P=0.000),而HI略低于dIMRT,差异无统计学意义(P>0.05)。Rapid Arc中膀胱的Dmean与D5%较d IMRT偏高(P=0.001)。小肠V20Gy中5F-0°-d IMRT高于Rapid Arc(P=0.041),V30 G y中5F-0°-d IMRT低于Rapid Arc低于5 F-180°-d IMRT。双侧股骨头的Dmean、D5%、V20Gy、V30Gy,5F-0°-d IMRT显著高于Rapid Arc和5F-180°-d IMRT,V40 G y则低于两者(P=0.004)。相对于Rapid Arc来说,B-P的V5Gy、V10Gy高于d IMRT(P<0.05),V15Gy略低于dIMRT,差异无统计学意义,V20Gy、V30Gy则显著低于d IMRT(P<0.05)。Rapid Arc的MU(382±16)分别较5F-0°-d IMRT(940±177)和5F-180°-d IMRT(956±144)减少了59%、60%(P=0.000)。Rapid Arc的TT(152±4)s分别较5F-0°-d IMRT(247±21)s和5F-180°-d IMRT(251±20)s减少了38%、39%(P=0.000)。结论 Rapid Arc计划可以达到或优于d IMRT计划的靶区剂量分布,能更好地保护小肠和双侧股骨头,并且具有较少MU与TT的优势,减少了治疗中不确定性因素的影响及患者不适感。
