眼内异物手术治疗体会

Clinical analysis of treatment for intraocular foreign body extraction operation

目的分析不同的手术方法治疗眼外伤后眼内异物的手术效果。方法对35例（均为单眼受伤）眼内异物的临床病例进行回顾性分析。结果35例异物取出手术成功后并保住眼球（100％），术后视力大部分有提高，22只眼行巩膜外路电磁铁吸出异物，13只眼采用玻璃体切除取出异物。其中31例并发外伤性白内障（31／35，88％），11例并发玻璃体积血（11／35，31％），2例并发视网膜脱离（2／35，2％），3例继发性青光眼（3／35，8％）。结论赤道前后、定位准确的磁性异物，以巩膜外路磁吸术为宜。对于合并玻璃体混浊的眼内异物、后极部异物需行眼玻璃体切除术。

Objective To evaluate the effect of surgery in cases of ocular trauma and intraocu lar foreigm bodies （IFBs）. Methods The Clinical data of 35 cases of intraocular foreign bodies were analyzed retrospectively. Results Successful operation was achieved in 35 eyes （100%）. The postoperative visual acuity of the most patients had varying degrees of rise. Twentytwo eyes under went electromagnet epispastic foreign body by externalroute surgery, and 13 eyes underwent vitrecto my extraction. In 35 cases, all foreign bodies were removed successfully. Among them the complica tions included traumatic cataract in 31cases （31/35, 57%）, veitreous hemorrhage in 11 cases （11/35, 31%）, retinal detachment in 2 cases （2/35, 2%） and secondary glaucoma in 3 cases （3/35, 8%）. Conclusions Electromagnet epispastic foreign body by externalroute surgery is suitable for the accu rate positioning magnetic foreign body before the equator. Vitrectomy is more favorable for the ex traction of intraocular foreign bodies and the posterior polar ones with lens turbidity and vitreous opacity. Ultimately the possibility whether the eye can reconstruct has certain relevance with the in tegrity of the foreign body＇s removal
