局麻腹膜前间隙疝修补术治疗老年腹股沟疝的临床研究

Clinical study on local anesthetic preperitoneal herniorrhaphy in the treatment of inguinal hernia in elderly

目的分析局麻腹膜前间隙疝修补术治疗老年腹股沟疝的临床疗效。方法选择2012年1月～2014年10月我院住院老年（60岁以上）腹股沟疝患者120例,采用随机数字表法分为观察组和对照组各60例,对照组行腹腔镜疝修补术治疗,观察组行局麻腹膜前间隙疝修补术治疗。比较两组手术情况。结果术后（4h、8h、12h、24h）两组NRS评分比较,差异无统计学意义（t=0.806,0.864,0.945,0.605,P〉0.05）,并发症率明显低于对照组,差异有统计学意义（x^2=5.551,P〈0.05）。观察组手术时间和住院费用明显低于对照组,差异有统计学意义（t=10.014,49.503,P〈0.01）。结论局麻腹膜前间隙疝修补术治疗老年腹股沟疝能有效改善患者临床症状,手术安全、并发症少、医疗费用低,值得基层医院推广应用。

Objective To analyze the clinical effect of local anesthetic preperitoneal herniorrhaphy in the treatment of inguinal hernia in elderly. Methods 120 elderly（60 years old） patients with inguinal hernia were selected from 2012 January to 2014 October in our hospital, randomly divided into observation group and control group,60 cases in each group, patients in the control group received laparoscopic hernioplasty, patients in the observation group received local anesthetic preperitoneal herniorrhaphy.Operation condition in the two groups was compared. Results After operation（4h, 8h, 12 h, 24h）,there was no significant difference in the two groups about NRS scores（t=0.806,0.864, 0.945,0.605, P〉0.05）. The rate of complications was lower, there was statistically significant（x^2=5.551,P〈0.05）.Operation time and cost of hospitalization was significantly lower,there was statistically significant（t=10.014, 49.503, P〈0.01）. Conclusion Local anesthetic preperitoneal herniorrhaphy in the treatment of inguinal hernia in elderly can improve the clinical symptoms of patients,with fewer complications,operation safety,and low medical cost, it is worthy of popularization and application in basic hospital.
