CT及MRI结合随访观察对诊断多房囊性肾癌价值的初步研究

A primary study of the value of CT and MRI combined with follow-up in diagnosis of multilocular cystic renal cell carcinoma

目的探讨CT及MRI结合随访观察诊断多房囊性肾癌的价值。方法回顾性分析31例经手术病理证实的多房囊性肾细胞癌患者的CT、MRI表现。结果31例共31个病灶，均为多房囊性病灶，边界光滑、囊壁及分隔增厚，部分病灶表现为厚薄不均。15例伴附壁结节，5例见囊壁钙化，6例诊断囊性占位患者随访观察中出现囊壁、分隔增厚、囊液密度不均匀增高等征象，最终经手术病理证实为多房囊性肾癌。结论CT及MRI检查结合随访观察对提高多方囊性肾癌的术前正确诊断率具有重要的临床意义。对于可疑肾囊性占位的病变若在随访观察中出现壁结节或囊壁增厚等征象是应高度怀疑多方囊性肾癌的可能。

Objective To investigate the value of CT and MRI combined with follow-up in diagnosis of muhilocular cystic renal cell carcinoma （MCRCC） to increase the accuracy diagnosis. Methods Thirty-one cases of MCRCC confirmed by operation and pathology were retrospectively studied based on the CT and MRI imaging data. Results Totally 31 lesions were found in 31 patients. All the lesions were composed of smooth-sharp border, multiple cyst and septa, with mural nodule in 15 cases and calcification in 5 cases. Six cystic lesions were diagnosed as muhilocular cystic renal cell carcinoma with imaging features of thickeni.ng capsule wall, septa and intracystic fluid component of higher density than water during follow-up. Conchmions CT and MRI findings of muhilocular cystic carcinoma of kidney have their own characteristics, CT and MRI combined with follow-up can play an important role in the diagnosis of muhilocular cystic renal cell carcinoma. Thickening capsule wall, septa, or higher density of the cystic lesion indicate canceration during follow-up.
