VEGF与IGF-I在EMs患者中表达及临床应用价值分析

目的：探讨血管内皮生长因子（ VEGF）及胰岛素样生长因子－Ⅰ（ IGF－Ⅰ）在子宫内膜异位症（ endometriosis ，EMs）中的表达情况及其临床应用价值。方法：选取68例经手术病理确诊为EMs的患者（其中Ⅰ期14例，Ⅱ期16例，Ⅲ期22例，Ⅳ期16例）为EMs组，选取60例非EMs患者（包括卵巢囊肿、输卵管结扎及不孕症行腹腔镜检查的患者）为非EMs组，另选取60例行妇科检查的正常妇女为对照组。采用ELISA法测定各组血清 VEGF、IGF－Ⅰ水平。应用受试者特异性曲线（ ROC ）分析VEGF与IGF－I在EMs患者中应用价值。结果：EMs组患者血清VEGF、IGF－I水平显著高于非EMs组及对照组（ P＜0．05），而非EMs组与对照组血清VEGF、IGF－I水平无统计学差异（ P＞0．05）。Ⅲ～Ⅳ期EMs患者血清VEGF、IGF－I水平显著高于Ⅰ～Ⅱ期（ P＜0．05）。经ROC分析显示，VEGF联合IGF－I检测时灵敏性、特异性、阳性预测值、阴性预测值高于各单项检测（ P＜0．05）。结论：血清VEGF、IGF－I表达水平升高与EMs的发生及病情的进展具有密切的关系，两者在EMs患者中可能通过协同表达促进病情的发生发展。通过联合检测可提高其临床诊断价值，有助于对EMs患者病情评估。

Objective： To investigate the expression and clinical application of vascular endothelial growth factor （VEGF} and insulin-like growth factor - I （IGF- I ） in endometriosis {（endometriosis, EMs）.Method： 68 cases of EMs as the observation group, 60 cases of non patients with EMs as control group, and the other 60 women of routine gynecological examinations as control group.The levels of VEGF, IGF- I of each groups were determinated. The VEGF and IGF-I value in EMs patients were applicated with specific subjects curve （ROC）.Result：The levels of VEGF, IGF-I were significantly higher than EMs andcontrol group （ P 〈0.05 ）, but the levels of VEGF, IGF-I of not EMs groups and control group were no signifi-cant difference （ P〉 0.05） .The levels of VEGF of Ⅲ～ Ⅳ EMS were higher than Ⅰ～ Ⅱ period （ P 〈0. 05）. The sensitivity, specificity, positive predictive value, negative predictive value of VEGF joint IGF-I were higher than the individual detection with ROC（ P 〈0.05）. Conclusion： Serum VEGF, IGF-I expres- sion levels increased disease progression EMs machine has a close relationship with both patients may in EMs promote the development of the disease through collaboration. Through through joint detection can im-prove their diagnostic value of clinical, patient assessment EMs contribute to the disease.
