血清PCT和hs-CRP检测在AECOPD患者中的临床价值

Clinical Value of Serum PCT and hs-CRP Assays in AECOPD Patients

目的:探讨血清降钙素原(PCT)和超敏C反应蛋白(hs-CRP)检测在慢性阻塞性肺疾病急性加重期(AECOPD)患者加重病因判定中的临床价值。方法:选择慢性阻塞性肺疾病(COPD)稳定期患者50例(稳定组)和加重期患者50例(加重组),测定血清PCT和hs-CRP。结果:加重组患者血清PCT和hs-CRP水平均高于稳定组,差异有统计学意义(P<0.05)。感染组患者血清PCT和hsCRP水平均高于非感染组,差异有统计学意义(P<0.05)。判定AECOPD患者是否由感染因素加重,血清PCT的敏感性87.50%、特异性72.22%、准确性82.00%,血清hs-CRP的敏感性90.63%、特异性66.66%、准确性82.00%,二者联合的敏感性96.88%、特异性94.44%、准确性96.00%。结论:COPD患者急性加重期血清PCT和hs-CRP水平升高,因感染原因加重者血清PCT和hs-CRP水平更高,血清PCT联合hs-CRP有利于判定COPD加重的原因。

Objective：To investigate the clinical value of serum procalcitonin （ PCT） and hypersensi-tive C reaction protein （ hs-CRP ） assays in judgment of causes of exacerbation in acute exacerbation phase of chronic obstructive pulmonary disease （AECOPD） patients.Method：50 patients in stable phase of COPD （stable group） and another 50 patients in exacerbation phase of COPD （exacerbation group） were selected, and the serum PCT and hs-CRP contents were assayed .Result：The serum PCT and hs-CRP levels in the patients of the exacerbation group were both higher than those of the stable phase , and the differences were statistically significant （ P＆lt;0.05） .The serum PCT and hs-CRP levels in the patients of infection group were both higher than those of infection-free group, and the differences were statistically significant （P〈0.05）. When judgment of whether the AECOPD patients were exacerbated because of infection , the sensitivity, spe-cificity and accuracy of serum PCT level were 87.50%, 72.22%, and 82.00%, respectively;those of serum hs-CRP level were 90.63%, 66.66%, and 82.00%, respectively;and those of the two in combination were 96.88%, 94.44%, and 96.00%, respectively.Conclusion：In acute exacerbation phase of COPD , the serum PCT and hs-CRP levels are both increased , and in the patients who were exacerbated because of infection , the serum PCT and hs-CRP levels were even higher .Serum PCT assay combined with serum hs-CRP assay is conducive to judgment of causes of exacerbation of COPD .
