末梢灌注指数变异率评价感染性休克复苏效果的临床研究 被引量:2

摘要 目的探讨末梢灌注指数变异率(△PFI)评价感染性休克患者预后的价值。方法选择感染性休克患者50例,行早期目标导向治疗(EGDT),治疗后根据脓毒性休克指南推荐6 h液体复苏标准,将患者分为复苏达标组(28例)和未达标组(22例),比较两组EGDT 3 h、6 h时△PFI的差异。计算达标组3 h△PFI均数,根据该均数将50例患者分为灌注改善组14例,未改善组36例,比较两组3 h、6 h乳酸清除率及病死率的差异。结果达标组在EGDT 3 h、6 h时△PFI均明显高于未达标组(P<0.05);灌注改善组、未改善组EGDT 3 h时乳酸清除率比较,差异无统计学意义(P>0.05),6 h时灌注改善组乳酸清除率明显高于未改善组(P<0.05)。结论△PFI能早期动态评价感染性休克复苏效果,EGDT 3 h时△PFI≥39%可作为判断感染性休克患者预后的指标。
