急诊标准操作规范救护模式在群体伤病员院内救治中的应用

Application of Emergency SOP Model in the Rescue of Urban Mass Casualty Incidents

目的介绍急诊标准操作规范(standard operation procedures,SOP)救护模式的编写原则与影响因素,分析该模式在城市突发群体伤救护中的运用效果。方法急诊SOP程序文件以作业指导书的形式对人员物资、伤员预检、诊断治疗、院内协调、预案演练等环节设置了标准化流程,进行规范化管理。结果在2011年上海地铁事故突发群体伤救治过程中,急诊SOP救护模式获得充分应用,统计显示,国际通用的病员辅助检查率、重症伤员误诊率、急诊科3h内可用ICU床位数和住院患者24h内手术率等指标均与国外相似规模群体伤救治的文献结果相仿。结论急诊SOP救护模式能够有效提高突发群体伤的院内急救效率,是一种有推广价值的管理模式。

Objective To introduce the principles and influencing factors of emergency SOP model and to analyze its application effect in urban mass casualty incidents.Methods The SOP program was built on the form of guidance manual to set a standard procedure including personnel and material preparation,pretest of patients,diagnosis and treatment,cooperation among hospital,emergency plan and practice.Results Our emergency SOP management model was fully implemented in the management of Shanghai metro mass casualty incident of 2011.The internationally applied patients assisted assessment rate,misdiagnosis rate of severe injuries,ICU beds available during first 3hours and operation rate of inpatients within 24 hours were all comparable to those reported overseas.Conclusion This SOP model is capable to manage mass casualty incidents efficiently,which is worth to be popularized.
