急性上消化道出血后贫血患者EPO与贫血程度的相关性 被引量:2

Correlation between erythropoietin level and degree of anemia in patients with acute upper gastrointestinal bleeding

摘要 目的:探讨、分析急性上消化道出血所致的贫血患者促红细胞生成素(erythropoietin,E P O)在不同时间段的变化以及贫血程度与EPO之间的关联.方法:选择在2012-06/2014-06湖北医药学院附属人民医院收治的70例急性上消化道出血所致的贫血患者作为研究对象,将其作为观察组,选择同期70例健康体检人群作为对照组,在发病后第1、3、7天测定观察组患者的EPO水平,并与健康人群相比较,比较不同程度贫血患者的EPO水平.结果:观察组患者的EPO水平在发病后第1、3、7天呈先上升后下降的趋势,E P O值在发病后第3天达峰值,与对照组相比,差异有统计学意义(P<0.05);发病后第1、7天的EPO值均高于对照组,但差异不具有统计学意义(P>0.05);在发病后第1、3、7天,不同程度贫血组中,重度贫血组患者的EPO值最高,明显高于其他两组的贫血患者,差异均有统计学意义(P<0.05).结论:急性上消化道出血后,患者的EPO水平增高,且随着时间变化而变化,同时EPO的水平和急性上消化道出血后患者的贫血程度密切相关,轻中度贫血时,EPO水平变化不明显,在重度贫血时,EPO水平会明显升高,有着重要的临床参考意义. AIM:To investigate the changes of erythropoietin(EPO) in patients with anemia caused by acute upper gastrointestinal bleeding(AUGIB) in different time periods,as well as the association between the degree of anemia and EPO.METHODS:Seventy patients with anemia caused by AUGIB treated from June 2012 to June 2014 at Affiliated Hospital of University of Medicine served as an observation group,and 70 healthy people served as a control group.The levels of EPO in patients in the observation group were measured at the first,third and seventh day after the onset and compared with those in the healthy controls.The levels of EPO in patients with different degrees of anemia were compared.RESULTS:The levels of EPO in the observation group patients showed an initial rising and then a downward trend from the first to the third and seventh day after the onset.The value of EPO peaked on the third day.Compared with the control group,the level of EPO on day 3 was significantly higher(P〈0.05).EPO values were higher than those in the control group on days 1 and 7,but the difference was not statistically significant(P〉0.05).The values of EPO in patients with severe anemia were significantly higher than those in patients with moderate or mild disease on days 1,3 and 7(P〈0.05).CONCLUSION:In patients with AUGIB,EPO levels change over time.The degree of anemia in patients with AUGIB is closely related to the level of EPO.In patients with mild to moderate anemia,EPO levels do not change significantly,but EPO levels significantly increase in severe anemia.
