泮托拉唑应用于预防创伤后应激性溃疡的临床价值

Pantoprazole for prevention of post-traumatic stress ulcer

目的:分析泮托拉唑应用于预防创伤后应激性溃疡的临床价值.方法:选取2012-01/2014-01南阳市中心医院接收治疗的外科重症及大手术患者108例,采用随机数表法将108例患者分为观察组和对照组,每组54例.给予观察组患者泮托拉唑,给予对照组患者西咪替丁.对比治疗2w k后两组患者的发生率和手术前后的胃液p H值.结果:观察组患者中有2例患者发生应激性溃疡,发生率为3.71%.对照组患者中有11例患者发生应激性溃疡,发生率为20.38%.比较两组间差异具有统计学意义(P<0.05).术前观察组患者和对照组患者的胃液p H值分别为2.00±0.22和2.00±0.23,术后观察组患者和对照组患者的胃液p H值分别为2.00±0.23和7.20±1.10.手术前后观察组患者的胃液p H值均高于对照组患者,比较两组间差异具有统计学意义(P<0.05).结论:泮托拉唑较西咪替丁预防创伤后应激性溃疡的临床疗效较好,能够显著降低应激性溃疡的发生率.

AIM：To assess the value of pantoprazole in the prevention of post-traumatic stress ulcer.METHODS：One hundred and eight patients who underwent major surgery or surgical critical patients treated from January 2012 to January 2014 at Nanyang Central Hospital were randomly divided into either an observation group or a control group,with 54 cases in each group.The observation group was given pantoprazole,and the control group was given cimetidine.After two weeks of therapy,the incidence of stress ulcer and gastric pH were compared for the two groups.RESULTS：Two cases of stress ulcer developed in the observation group,and the rate was 3.71%.Eleven cases of stress ulcer were observed in the control group,and the rate was 20.38%.The rate of stress ulcer differed significantly between the two（P〈0.05）.Preoperative gastric pH values in the observation group and control group were2.00 ± 0.22 and 2.00 ± 0.23,respectively,and corresponding postoperative values were 2.00± 0.23 and 7.20 ± 1.10.Gastric pH values both before and after surgery in the observation group were significantly higher than those in the control group（P〈0.05）.CONCLUSION：Pantoprazole compared with cimetidine is effective in the prevention of traumatic stress ulcer and can significantly reduce the incidence of stress ulcer.
