老年高血压患者中心动脉压与肱动脉压的对比分析

摘要 目的对老年高血压患者的中心动脉压(CAP)和肱动脉血压进行对比分析,观察CAP与肱动脉压的差异。方法采用动脉脉搏波分析仪测量152例老年高血压患者的CAP,并与肱动脉血压进行比较分析。结果老年高血压患者的肱动脉收缩压(SBP)、脉压(PP)显著高于中心动脉的收缩压(c SBP)、脉压(c PP)[(134.34±16.38)mm Hg比(116.21±14.71)mm Hg;(61.16±14.74)mm Hg比(37.55±7.92)mm Hg],而肱动脉舒张压(DBP)则显著低于中心动脉的舒张压(c DBP)[(73.03±9.66)mm Hg比(79.34±11.23)mm Hg],差异均有统计学意义(P<0.01)。与Ⅰ级高血压患者相比,Ⅲ级高血压患者的SBP、PP、中心动脉增强压(AP)和增强指数(AIx)明显增高,差异均有统计学意义(P<0.05)。结论肱动脉血压与CAP有显著差异,外周肱动脉血压不能替代CAP。 Objective To compare the difference between central aortic pressure and brachial artery pressure in elderly patients withhypertension.Methods Onehundred and fifty-two elderly patients withhypertension were enrolled this study.The central aortic pressure of each patient was measured by Sphygmo Cor(Australia),respectively,and compared with brachial artery pressure.Results The brachial artery systolic pressure and pulse pressure werehigher than central aortic systolic pressure and pulse pressure[(134.34 ± 16.38) mm Hg vs.(116.21 ± 14.71) mm Hg,(61.16 ± 14.74) mm Hg vs.(37.55 ± 7.92) mm Hg]and the difference was significant(P 0.01).But the brachial artery diastolic pressure was lower than central aortic diastolic pressure,the difference was significantly statistically(P 0.01).Compared withhypertension patients of grade-Ⅰ,systolic pressure and pulse pressure of brachial artery,augmentation pressure and augmentation index of central aortic were significantlyhigher inhypertension patients of grade-Ⅲ(P 0.05).Conclusion There is significant difference between central aortic pressure and brachial artery pressure,it is necessary to measure the central aortic pressure for diagnosis and treatment ofhypertension in elderly patients.
