微型营养评价法用于80岁以上高龄患者营养评估 被引量:7

摘要 目的探讨微型营养评价法(MNA)用于80岁以上高龄患者营养评估的效果。方法选取老干部病房80岁以上高龄患者120例,分别用台湾修订版MNA和原版MNA进行营养评估,并测定各项传统营养指标。结果台湾修订版MNA营养评估结果显示,营养不良患者37例(30.8%),营养不良危险患者42例(35.0%),两者比率均高于原版MNA的28.3%、33.3%,但差异无统计学意义(P>0.05)。台湾修订版MNA营养评估的3个营养分组之间体质量指数(BMI)、小腿围(CC)、三头肌皮褶厚度(TSF)、血清白蛋白(ALB)、转铁蛋白(TRF)、前白蛋白(PAB)、血红蛋白(HB)、总淋巴细胞计数(TLC)比较差异有统计学意义(P<0.05),三酰甘油(TG)、总胆固醇(TC)比较差异无统计学意义(P>0.05),台湾修订版MNA和传统营养指标BMI、上臂围(MAC)、CC、TSF、ALB、TRF、PAB有显著相关性(P<0.05)。结论台湾修订版MNA适用于80岁以上高龄患者营养评估,80岁以上高龄患者营养不良发生率较高,应当尽早进行营养干预。 Objective To investigate the efficacy of themini nutritional assessment(MNA) for nutritional assessment in elderly patients over 80 years old.Methods 120 patients over 80 years old received nutritional assessment by the Taiwan revised MNA and the original MNA respectively,the classical nutritional markers were measured in each patient.Results According to nutritional assessment result of Taiwan revised MNA,37 cases of patients were with malnutrition(30.8%),42 cases of patients were with malnutrition risk(35%),the ratio washigher than that of original MNA,but the differences was not statistically significant(P 0.05).According to nutritional assessment result of Taiwan revised MNA,the differences of BMI,CC,TSF,ALB,TRF,PAB,HB,TLC between the three nutrition group were statistically significant(P 0.05),the differences of TG,TC between the three nutrition group were not statistically significant(P 0.05),Taiwan revision MNA and BMI,MAC,CC,TSF,ALB,TRF,PABhad significant correlations(P 0.05).Conclusion Taiwan revised MNA is suitable for nutritional assessment in elderly patients over 80 years old,and those peoplehave ahigher incidence of malnutrition that should be receive nutritional intervention early.
