2型糖尿病及其合并冠心病患者红细胞分布宽度的变化 被引量:7

The changes of red blood cell distribution width in type 2 diabetes mellitus and diabetes mellitus complicated with coronary heart disease patients

摘要 目的探讨红细胞分布宽度在2型糖尿病及其合并冠心病患者中变化的临床意义。方法将212例2型糖尿病患者按照是否合并冠心病分为两组,单纯糖尿病组102例,糖尿病合并冠心病组110例,另选非糖尿病患者128例为对照组。测定各组红细胞分布宽度、空腹血糖的水平,并进行比较及相关分析。再将所有糖尿病患者以红细胞分布宽度13%为界限分为两组,比较两组冠心病的发生率。结果对照组、单纯糖尿病组、糖尿病合并冠心病组的红细胞分布宽度、血糖呈逐渐升高趋势[(12.51±0.36)%比(12.72±0.56)%比(13.13±0.53)%;(5.00±0.41)mmol/L比(6.57±1.03)mmol/L比(7.07±1.97)mmol/L],且各组间均差异有统计学意义(P<0.05),红细胞分布宽度与血糖呈正相关(r=0.275,P=0.000)。红细胞分布宽度高的糖尿病组冠心病发生率明显高于红细胞分布宽度低的糖尿病组(67.3%比36.2%,χ2=4.603,P=0.000)。结论红细胞分布宽度对判定2型糖尿病的病情程度和大血管并发症的早期诊断有一定的临床参考价值。 Objective To observe the levels of red blood cell distribution width(RDW) in type 2 diabetes mellitus(DM) and DM complicated with coronaryheart disease(CHD).Methods 212 patients of type 2 diabetes mellitus were selected and divided into two groups according to whether complicated with CHD.There were 102 patients in DM group and 110 patients in DM complicated with CHD,128 patients without diabetes mellitus were selected as control group.The RDW and glucose level were compared among different groups and the correlation between them were analyzed.All the DM patients were divided into two groups according to the cutoff point of RDW at 13%.The morbidity of CHD was compared between the two groups.Results The RDW,glucose level were gradually increased in control group,DM group and DM complicated with CHD group[(12.51 ± 0.36) % vs(12.72 ± 0.56) %vs(13.13 ± 0.53) %;(5.00 ± 0.41) mmol/L vs(6.57 ± 1.03) mmol/L vs(7.07 ± 1.97) mmol/L].There were significant differences of RDW and glucose level among the three groups.RDW was positive correlated with glucose level(r = 0.275,p = 0.000).The morbidity of CHD inhigh RDW DM group washigher than that in low RDW DM group(67.3% vs 36.2%,χ2= 4.603,P = 0.000).Conclusion The measurement of RDW play an important role in the evaluation of the severity of diabetes mellitus and the early diagnosis of macrovascular complications.
