超声引导下置管引流术治疗盆腔恶性肿瘤术后淋巴囊肿的临床应用价值

Value of ultrasound-guided catheter drainage of postoperative lymphocyst

目的：探讨超声引导下经腹置管引流术治疗盆腔恶性肿瘤术后淋巴囊肿的临床应用价值。方法在经腹部超声引导下，对112例盆腔恶性肿瘤患者术后所形成的143个淋巴囊肿经皮穿刺置入8．5F Cook外引流管持续引流淋巴囊液。结果143个淋巴囊肿均一步法一次成功穿刺并置入8．5F Cook外引流管，未出现与导管穿刺相关并发症。所有病灶均成功治疗，完全消失142例，治愈率达99．3％，仅1例于治疗3个月后复发，复发率不足1％。引流时间3～15 d，平均5 d。引流量270～3200 ml ，平均812 ml。结论超声引导下经腹盆腔淋巴囊肿行穿刺置管治疗简单易行，患者痛苦小，并发症少，疗效确切，复发率低，安全性高，是盆腔淋巴囊肿的最佳治疗方法之一。

Objective To assess the value of transabdominal ultrasound-guided catheter drainage of postoperative lymphocysts in patients with pelvic malignancy. Methods 143 postoperative lymphocysts in 112 patients with pelvic malignancy were drained percutaneously using 8.5F Cook catheters under ultrasound guidance. Results The procedure was successful in all patients without complications. 142 lymphocysts were completely healed with cure rate of 99.3%. Only one recurred after three months. The duration of drainage ranged from 3 to 15 days （average：.5 days） totaling 270-3200 ml（average：.812 ml）. Conclusion Transabdominal ultrasound-guided percutaneous drainage of pelvic lymphocysts is simple and safe with less pain , fewer complications , high cure rate , and low recurrence rate.
