多烯磷脂酰胆碱治疗慢性乙肝并发NAFLD的效果观察

Observation on effect of polyene phosphatidylcholine on chronic hepatitis B complicated with NAFLD

目的探究多烯磷脂酰胆碱（PPC）治疗慢性乙肝并发非酒精性脂肪肝（NAFLD）的临床效果。方法将我院2013年2月～2014年2月间收治的92例慢性乙肝并发NAFLD患者随机分为两组，每组各46例。研究组采用多烯磷脂酰胆碱联合复方甘草酸苷治疗，对照组仅采用复方甘草酸苷治疗。比较两组治疗后肝功能、血脂水平和临床疗效。结果与治疗前比较，治疗后两组谷丙转氨酶（ALT）、谷草转氨酶（AST）、谷氨酰转肽酶（GGT）、甘油三酯（TG）、血清总胆固醇（TC）水平均显著降低（P〈0．01），但研究组较对照组降低更显著（均P〈0．01）。研究组治疗有效率为76．09％（35／46），显著高于对照组的32．61％（15／46）（P〈0．01）。结论多烯磷脂酰胆碱联合复方甘草酸苷治疗慢性乙肝并发NAFLD的效果好。既能更显著恢复肝功能，又能显著降低血脂水平，并提高临床疗效，值得临床推广使用。

Objective To explore the clinical curative effect of polyene phosphatidylcholine （PPC） on chronic hepatitis B complicated with non-alcoholic fatty liver disease （NAFLD）. Methods 92 patients with chronic hepatitis B complicated with NAFLD to receive treatment in our hospital from Feb. 2013 to Feb. 2014 were randomly divided into two groups （n = 46 per group）. PPC combined with compound Glycyrrhizin was used in the research group while only compound Glycyrrhizin was used in the control group. And then, the liver function, blood fat level and clinical curative effect were compared. Results Compared with the situation before treatment, the alanine aminotransferase （ALT） and aspartate aminotransferase （AST）, glutamyl transpeptidase （GGT）, triglyceride （TG）, and serum total cholesterol （TC） levels in the two groups after treatment significantly decreased （ all P 〈 0.01 ）, but the levels in the research group decreased more significantly than those in the control group （P 〈 0.01 ）. The effective rate of the research group was 76.09% （35/46）, significantly higher than that in the control group 32.61% （15/46） （P 〈 0.01）. Conclusion PPC combined with compound Glycyrrhizin in the treatment of chronic hepatitis B complicated with NAFLD has significant effect, not only recovering the liver function more significantly, but also reducing the blood fats level significantly, as well as improving the clinical efficacy, so it is worthy of promotion and clinical use.
