西地那非改善反复移植失败患者子宫内膜血流的研究 被引量:19

Sildenafil improves endometrial blood flow in patients with repeated implantation failure

摘要 目的探讨西地那非在反复移植失败患者中改善子宫内膜血流及提高临床妊娠率的作用。方法选择2011年6月至2012年12月在本生殖医学中心接受胚胎解冻移植(frozen thawed embryo transfer,FET)助孕,且彩色多普勒能量图提示子宫内膜血流不良的反复胚胎移植失败妇女,共221个周期。应用西地那非治疗1-2个疗程,分析用药前后子宫内膜血流改善情况,并随访FET结局。结果 221个周期中,7个周期因患者服用西地那非副反应中途放弃,214个周期进行了胚胎移植,临床妊娠率36.92%(79/214),血流改善率64.02%(137/214),药物不良反应率15.84%(35/221)。214个周期中,用药前,B超示无血流信号、仅子宫内膜下可及血流信号、子宫内膜下及内膜内均可及血流信号者所占比例分别为49.53%(106/214)、50.47%(108/214)和0,用药后分别为12.15%(26/214)、31.78%(68/214)和56.07%(120/214),差异有统计学意义(P〈0.01)。结论西地那非可明显改善反复移植失败患者的子宫内膜血流情况,提高患者的胚胎种植率及临床妊娠率。 Objective To study the role of sildenafil in improving the endometrial blood flow and pregnancy rate in repeated implantation failure(RIF)patient.Methods We analyzed RIF patients with sub-endometrial blood flow only or no blood flow detected in 221frozen-thawed transfer(FET)cycles in our hospital from 2011 to 2012.Sildenafil was used and endometrial blood flow was measured one day before transferring.Results In the 214 cycles of FET,the clinical pregnancy rate was 36.92%(79/214).Before using sildenafil,the ratio of undetectable blood flow,sub-endometrial blood flow,and both endometrial and sub-endometrial blood flow was 49.53%(106),50.47%(108/214)and 0.After using sildenafil,the ratio turned to be 12.15%(26/214),31.78%(68/214)and 56.07%(120/214).The changes was significant.The blood flow of endometrium was improved in 64.02%(137/214)cycles.Conclusion Sildenafil can improve endometrial blood flow and pregnancy rate in repeated implantation failure(RIF)patient significantly.
