急性心肌梗死患者血浆B型利钠肽和基质金属蛋白酶9变化与心功能关系探讨 被引量:5

摘要 目的:探讨急性心肌梗死(AMI)患者血浆B型利钠肽(BNP)和基质金属蛋白酶9(MMP-9)的变化及与心功能的关系。方法:选择首都医科大学大兴医院就诊的急性ST段抬高型心肌梗死65例研究对象为AMI组,选择同期住院的30例非冠心病患者为对照组。AMI患者于急诊冠脉介入治疗后抽血测定血浆BNP和MMP-9浓度,对照组于排除冠心病后抽血测定上述指标。AMI患者住院期间行超声心动图检查评价心功能。结果:AMI组患者血浆BNP、MMP-9浓度均明显高于对照组,P<0.05。AMI组患者血浆BNP浓度和MMP-9浓度呈正相关(r=0.383,P<0.05),BNP浓度与左室舒张末内径呈正相关(r=0.649,P<0.05),而MMP-9浓度与左室射血分数呈负相关(r=-0.466,P<0.05)结论:AMI患者血浆BNP和MMP-9浓度均明显升高,两者均与AMI后心脏功能变化有一定关系。 Objective: To investigate the change of plasma B-type natriuretic peptide (BNP) and Matrix Metalloproteinase-9 (MMP-9) and relation with cardiac function in patients with acute myocardial infarction(AMI).Methods:65 patients of acute ST elevation myocardial infarction who were in hospital in Daxing Hospital of the Capital Medical University were determined objects of study(AMI group). 30 patients who were discharged of coronary artery disease in hospital were established control group. The blood samples were drawn after primary percutaneous coronary intervention (PCI) in AMI patients and after discharge of coronary artery disease in control group. The plasma concentrations of BNP and MMP-9 were assayed. The cardiac function was evaluated by echocardiography in AMI patients during in hospital. Results: Patients in the AMI group,the concentration of plasma BNP and MMP-9 were signiifcantly higher than those in the control group,P〈0.05. The plasma concentration of BNP was positive correlation with that of MMP-9 (r=0.383,P〈0.05) and left ventricular diastolic diameter (r=0.649,P〈0.05) in AMI patients. The plasma concentration of MMP-9 was negative correlation with left ventricular ejection fraction (r=-0.466,P〈0.05).Conclusion:The plasma concentrations of BNP and MMP-9 were elevated obviously in patients with acute myocardial infarction. They had some relationships with the change of cardiac function after acute myocardial infarction.
