替吉奥联合放射治疗术后局部复发直肠癌的疗效观察

Efficacy of Tegafur combined with radiotherapy in the treatment of colorectal cancer local recurrence

目的探讨替吉奥联合放疗治疗手术后局部复发直肠癌的疗效,为直肠癌术后局部复发的治疗提供依据。方法选取2009年8月至2014年8月间收治的直肠癌术后局部复发患者136例,按照临床试验数字随机分组法分为两组,每组68例。对照组患者采用单纯的放射治疗,治疗组患者采用替吉奥联合放射治疗。观察两组患者的近期疗效及不良反应发生情况。结果治疗组患者的总有效率(RR)和疾病控制率(DCR)分别为82.4%和97.1%,明显优于对照组的63.2%和88.2%,差异有统计学意义(P<0.05)。两组患者治疗期间主要不良反应有白细胞下降、中性粒细胞下降及贫血,其中对照组患者白细胞和中性粒细胞下降的发生率分别为25.0%和22.1%,明显低于治疗组的47.1%和51.5%,差异有统计学意义(P<0.05)。结论替吉奥联合放疗治疗术后复发直肠癌疗效确切,不良反应轻微,无严重不良反应发生,可为复发直肠癌患者提供新的选择方案。

Objective To investigate the curative effect of Tegafur combined with radiotherapy in the treatment of postoperative local recurrence of rectal cancer. Methods From August 2011 to August2014,136 patients with recurrent rectal cancer in The Second People＇s Hospital of Dezhou were enrolled in the study. According to digital randomized clinical trial,patients were divided into two groups： Petients in the control group accepted simple radiotherapy treatment,and patients in the treatment group accepted Tegafur combined with radiotherapy（ 68 cases in each group）. Three months after the treatment,computed tomography（ CT） was applied to observe the short-term efficacy and adverse drug reactions. Results The total effective rate（ RR） and disease control rate（ DCR） in the treatment group were 82. 4% and 97. 1%,significantly better than those of the control group（ 63. 2%,88. 2%）,with statistically significant differences（ P〈0. 05）; the main toxic reactions during the treatment in both groups mainly are decrease of white blood cells,neutropenia and anemia,and the white blood cells and neutrophils decreased incidence rate of the control group were 25. 0% and 22. 1%,significantly lower than those of the treatment group（ 47. 1%,51. 5%）,and the difference was statistically significant（ P〈0. 05）. Conclusion Tegafur combined with radiotherapy in treatment of postoperative recurrence of rectal cancercan provide a new selection scheme for recurrent rectal cancer,with good curative effect and less adverse reactions.
