尿道淋巴瘤临床病理特征分析(附1例报告并文献复习)

Analysis of clinicopathological features of urethral lymphoma(Report of one case and literature review)

目的:提高对尿道淋巴瘤诊断和治疗的认识。方法:报道1例尿道淋巴瘤患者临床病理特征及治疗,结合文献讨论其临床表现、病理类型、鉴别诊断、病因、治疗及预后。结果:临床诊断尿道肉阜,行病变切除术。病理检查:尿道外口黏膜上皮下见弥漫增生的大淋巴样细胞浸润,免疫组化染色CD20阳性,诊断为弥漫性大B细胞淋巴瘤。术后R-CEOP方案化疗6周期,随访21个月无复发。结论:尿道淋巴瘤好发于女性,需与尿道肉阜、尿道癌等鉴别。病变局限于尿道者预后相对较好,播散性病变预后较差。

Objective： To improve the diagnosis and treatment of urethral lymphoma. Method： We reported the clinicopathological characteristics and treatment of a case of primary urethral lymphoma, reviewed the relevant literature, and discussed the clinical manifestation, pathological subtypes, differential diagnosis, etiology, treatment and prognosis. Result： Clinical diagnosis was urethral caruncle and the lesion was resected. Pathological examination revealed diffused large lymphoid cells infiltrated under the epithelium of urethral meatus. Immunohistochemical staining showed the tumor cells were positive for CD20. Pathological diagnosis was diffuse large B cell lymphoma. The patient was treated with six cycles of R-CEOP chemotherapy. No recurrence was found after 21 months of follow-up period. Conclusion： Urethral lymphoma generally occurs in women. It should be distinguished with urethral caruncle, urethral cancer and so on. Lesions confined to urethra have a relatively good prognosis, while disseminated diseases are associated with poor prognosis.
