经皮激光汽化减压术联合臭氧治疗腰椎间盘突出症的临床观察

Clinical Curative Effect of Combined Treatment of Percutaneous Laser Disc Decompression and Ozone Ablation on Lumbar Disc Herniation

目的观察经皮激光汽化减压术（Percutaneous laser disc decompression,PLDD）联合臭氧消融术（Ozone ablation,O3）治疗腰椎间盘突出症（Lumbar disc herniation,LDH）的临床疗效。方法回顾性分析2013年5-12月,LDH患者42例的临床资料,采用PLDD联合O3治疗后,用改良的Macnab评价标准判断治疗后3和6个月的临床疗效,采用疼痛视觉模拟评分法（visual analogue scale,VAS）观察患者疼痛的改变,并观察治疗前后CT片中突出物矢状径（H）与椎管矢状径（V）的比值（H/V）的改变。结果 PLDD联合O3治疗LDH疗效确切;治疗后3个月的临床优良率为66.7%、治疗后6个月临床优良率81.0%。治疗后3和6个月较治疗前疼痛明显减轻,差异具有显著意义（P〈0.05）。CT检查结果显示,治疗后6个月与治疗前比较,椎间盘可有效的回纳,差异具有显著意义（P〈0.05）。结论 PLDD联合O3疗效确切,是临床治疗LDH的有效方法。

Objective To observe the clinical curative effect of the combined treatment of percutaneous laser disc decompression（PLDD） and ozone ablation（O3） on lumbar disc herniation（LDH）.Methods Altogether 42 patients of LDH treated jointly with PLDD and O3 in Gansu Provincial Hospital of Traditional Chinese Medi-cine from May to December 2013 were selected.The clinical curative effect was observed with modified Macnab criteria,change of pain was assessed with visual analogue scale（VAS）,and changes to projection prominent sagittal diameter（H） and sagittal diameter of spinal canal（V） ratio（H / V） in CT film before and 3 and 6 months after the therapy were observed.Results PLDD combined with O3 was effective in treating LDH： the clinical efficiency was 66.7% three months after the treatment,and the clinical efficiency 81.0% six months after the treatment.The patients＇ pain relieved three and six months after the treatment,and the difference was significant（P〈0.05）.The intervertebral disc could effectively return six months after the treatment,and the difference was significant（P〈0.05）.Conclusions The combined treatment of PLDD and O3 is effective on LDH.
