摘要
目的:探讨多层螺旋CT冠状动脉成像在冠状动脉疾病中的应用。方法:收集冠状动脉粥样硬化性心脏病患者43例、冠状动脉支架植入术后患者9例、冠状动脉搭桥术后患者5例,共57例患者进行多层螺旋CT冠状动脉成像(MSCTA)并行冠状动脉三维重建,将其冠状动脉造影检查(DSA)与MSCTA结果进行比较。结果:MSCTA对冠状动脉狭窄显示的敏感性为80%以上,特异性可达90%以上,且冠状动脉狭窄的阴性预测值为94%左右,诊断的准确率为81%。结论:MSCTA对冠状动脉中度以上狭窄的阴性预测值较高,对冠状动脉中度以上狭窄具有重要的诊断价值,基本能够满足冠状动脉疾病初步诊断的影像学需求。
Objective: To explore the application of multislice spiral CT coronary angiography( MSCTA) in the diagnosis of coronary artery disease. Method: 57 cases of coronary artery disease were collected and given MSCTA and 3D reconstruction of coronary artery,which include 43 cases of coronary atherosclerotic heart disease,9 cases of coronary stent implantation,5 cases of coronary artery bypass grafting. And then the results of digital subtraction angiography of coronary artery and those of MSCTA were compared. Results: the sensitivity of MSCTA to the coronary artery stenosis is more than 80. 0% and the specificity can reach more than 90%; the negative predictive value of coronary artery stenosis is about 94.6%; the diagnostic accuracy is 81. 3%. Conclusion: The negative predictive value of coronary artery stenosis by MSCTA is higher,which has an important diagnostic value in moderate coronary artery stenosis and above. In general,MSCTA can basically meet the imaging needs of preliminary diagnosis of the coronary heart disease.
