支气管肺泡灌洗液液基细胞学检查在周围型肺癌诊断中的价值

Diagnostic value of liquid based cytology test in bronchoalveolar lavage fluid for peripheral lung cancer

目的探讨支气管肺泡灌洗液(Bronchoalveolar lavage fluid,BALF)液基细胞学检查(Liquid based cytology test,LCT)在周围型肺癌诊断中的价值。方法选取同时进行了BALF传统涂片细胞学(Conventional smear,CS)、LCT、组织病理学检查的88例周围型肺癌和28例肺部良性病变患者,比较CS和LCT诊断周围型肺癌的敏感度、特异度及两者在肺鳞癌、腺癌中的阳性率。结果 BALF-CS和LCT诊断周围型肺癌的敏感度分别为51.14%、68.18%,差异存在统计学意义(χ2=3.972,P<0.05);特异度分别为96.43%、100%,差异不存在统计学意义(χ2=0.168,P>0.05);与CS相比较,LCT在肺腺癌中的阳性率更高(χ2=6.986,P<0.01)。结论支气管肺泡灌洗液液基细胞学检查在周围型肺癌的诊断中可能具有较重要的价值。

Objective To evaluate the diagnostic value of liquid based cytology test（ LCT） in bronchoalveolar lavage fluid（ BALF） for peripheral lung cancer. Methods 88 patients with peripheral lung cancer and 28 patients with lung benign lesion were collected and received Conventional smear test（ CS）,LCT in BALF and histopathology examination. The sensitivity,the specificity and the positive rate in the peripheral squamous carcinoma or adenocarcinoma of lung were analyzed. Results The sensitivity of CS and LCT in BALF for the diagnosis of peripheral lung cancer were 51. 14% and 68. 18% respectively（ χ^2= 3. 972,P〈 0. 05）. The specificity was 96. 43% and100% respectively（ χ^2= 0. 168,P〉 0. 05）. For the diagnosis of peripheral adenocarcinoma of lung,the positive rate of LCT in BALF was significantly higher than that of CS（ χ^2= 5. 226,P〈 0. 01）. Conclusion The LCT in BALF has possibly more important value in the diagnosis of peripheral lung cancer.
