2014年山东省部分煤矿井下作业工人尘肺结核患病情况调查 被引量:10

摘要 目的调查山东省济宁、兖州、新泰、宁阳地区的6个矿区接尘工人尘肺结核的患病情况,为尘肺结核的防治提供依据。方法 2014年3~6月利用山东省6大矿区职业性健康检查普查资料,选择井下接尘工龄≥1年的在职、离退休人员3 318人作为观察对象,选择1 161名无接尘的地面工作人员作为对照。依据高千伏X线胸片征象,结合临床表现及既往病史,诊断结核病(主要为活动性肺结核),分析比较2组人群尘肺结核的患病情况等。结果本次调查中,尘肺合并肺结核的患病率为7.50%。井下接尘工人累计感染率比对照高(年龄标化后肺结核累计感染率分别为12.35‰、4.30‰)(P〈0.05)。结论井下接尘工人肺结核患病率高于地面作业对照人群;接尘工龄长短可能对接尘工人尘肺结核发生有一定的影响。 Objective To survey the prevalence of pneumoconiosis tuberculosis among some workers exposed to dust in Jining,Yanzhou,Xintai and Ningyang of Shandong province,so as to provide the basis for the prevention and treatment of pneumoconiosis tuberculosis. Methods Using the occupation health examination survey data,3 318 miners exposed to dust with work-age more than one year were selected as the objects of observation,while another 1 161 workers not-exposed to dust were selected as control.On the basis of X-ray,combined with clinical manifestations and past medical history,diagnosis of tuberculosis(mainly active pulmonary tuberculosis),analysis and comparison of two groups of pneumoconiosis tuberculosis morbidity. Results In the survey,the prevalence of pneumoconiosis combined with tuberculosis was7.50%.Underground workers exposed to dust had higher cumulative prevalence than in the control group(cumulative prevalence of tuberculosis was age-standardized 12.35‰,4.30‰),the difference was statistically significant(P〈0.05). Conclusion The TB prevalence rate among miners exposed to dust is higher than ground operation control group;The seniority to dust may have a certain impact on the incidence of pneumoconiosis combined with tuberculosis.
