关节镜下保留与不保留残端重建交叉韧带效果的Meta分析 被引量:1

摘要 目的比较关节镜下保留与不保留残端重建交叉韧带的效果。方法在Pub Med数据库、清华同方数据库(CNKI)、中国生物医学文献数据库(Sino Med)等数据库检索有关关节镜下保留与不保留残端重建交叉韧带效果的临床对照文献,采用Meta分析两种方法的疗效。结果共收集5个临床对照研究,Meta分析结果显示:对于后交叉韧带,保留残端组在Lysholm评分上与不保留残端组差异无统计学意义(P=0.074),而IKDC评分比较差异有统计学意义(P=0.001)。对于前交叉韧带,两种方法治疗后Lysholm评分比较差异有统计学意义(P=0.045);IKDC评分和KT-2000评分比较差异无统计学意义(P>0.05)。结论保留残端重建前、后交叉韧带比不保留残端有一定的优势,但不能完全替代不保留残端重建术。这两种方法的优劣仍需开展更多高质量、大样本的研究来证实。 Objective To compare the effect of arthroscopic anterior cruciate ligament reconstruction with or without remnant preserving technique. Methods The databases of PubMed, CNKI, and SinoMed were searched for the relevant studies. Meta-analysis was conducted to compare the effects. Results Five studies were included. Meta-analysis results showed that the Lysholm score of posterior cruciate ligament had no significant difference between two groups (P = 0.074), and there was significant difference with regard to the IKDC score ( P = 0. 045). As for anterior cruciate ligament, no significant difference was found between two groups in terms of IKDC scores and KT-2000 (P 〉 0.05). Conclusion Remnant preserving technique has some advantages for crueiate ligament reconstruction, and further study is needed to confirm whether it can entirely replace the non-remnant preserving technique.
