贻贝粘蛋白治疗烧伤后瘙痒的临床观察 被引量:7

摘要 目的观察贻贝粘蛋白治疗烧伤后创面愈合期和瘢痕增生期瘙痒的疗效。方法将无锡市第三人民医院烧伤科2013年1月至2013年12月收治的40例烧伤后创面愈合期或瘢痕增生期有瘙痒症状的患者随机分为试验组和对照组,每组各20例,分别使用贻贝粘蛋白和0.9%氯化钠溶液进行治疗,记录治疗前后的瘙痒评分、止痒效果及治疗后的瘙痒缓解起效时间、持续时间、缓解程度以及全身不良反应。结果试验组瘙痒平均评分由治疗前(9.3±0.5)分下降至治疗后(1.1±0.2)分,差异有统计学意义(t=2.26,P<0.05);对照组瘙痒平均评分由治疗前(9.2±0.6)分下降至治疗后(8.1±0.5)分,差异无统计学意义(t=1.28,P>0.05)。试验组的止痒有效率为100%,优于对照组的有效率25%,差异有统计学意义(χ2=1.32,P<0.05)。试验组平均止痒起效时间(12.0±1.3)min,对照组只有5例患者部分缓解,平均止痒起效时间(30±2.7)min,两组比较差异有统计学意义(t=10.31,P<0.05);试验组平均止痒持续时间(36±3.6)h,对照组平均止痒持续时间(0.5±0.2)h,两组比较差异有统计学意义(t=-3.17,P<0.05)。试验组与对照组治疗期间均未出现不良反应。结论贻贝粘蛋白治疗烧伤后瘙痒安全有效,为临床提供一种可行的治疗方法。 Objective To observe effect of mussel adhesive protein (MAP) on itching during wound healing. Methods Total 40 patients from department of bums, the third people's hospital of Wuxi between January 2013 and December 2013 were divided into the test group and the control group stochastically,and treated by MAP and 0. 9% sodium chloride solution respectively. The itching score, antipruritic effect, onset time, curative effect, duration time, alleviate degree and toxicity were recorded. Results The average itching score of test group reduced from 9.3±0.5 to 1.1± 0.2, the two means were significantly different ( t = 2.26, P 〈 0.05 ). The control group reduced from 9.2±0.6 to 8.1±0.5, the two means are not significantly different ( t = 1.28, P 〉 0.05 ). The efficiency of test group was 100% , better than that of the control group 25% , the difference was statistically significant (X2 = 1.32 ,P 〈 0.05 ). The average onset time was ( 12.0±1.3 ) rain and ( 30. 0±2.7 ) min for test group and control group respectively, the two means were significantly different ( t = 10. 31, P 〈 0.05 ). The duration time was (36.0±3.6) h and ( 0.5±0.2) h for test group and control group respectively, the two means were significantly different ( t = 3.17, P 〈 0.05 ). No adverse reactions were observed during the test group and control group treatment. Conclusion Mussel adhesive protein wound repair dressing can relieve itching of burn wound healing period, provides a feasible method for the clinical treatment.
