体外循环与非体外循环下冠状动脉旁路移植术对脑钠肽的影响 被引量:4

Influences of cardiopulmonary bypass-coronary artery bypass grafting and off-pump coronary artery bypass grafting on level of brain natriuretic peptide

摘要 目的探讨体外循环及非体外循环下冠状动脉旁路移植术(CABG)对心功能的影响。方法选择2013年6月至2014年10月期间在空军总医院心血管外科行CABG的患者40例,女性14例,男性26例。按是否应用体外循环机随机分为体外循环组(CPB-CABG组)和非体外循环组(OPCABG组),每组各20例。于术前一天、术后12 h、24 h、36 h采集血浆标本,检测脑钠肽(BNP)含量。结果两组术后12h、24 h、36 h较术前血浆BNP水平均明显升高,差异有统计学意义(P均<0.05)。与OPCABG组相比,CPB-CABG组术后12 h[(138.0±42.6)pg/m L vs.(266.0±52.1)pg/m L]、24 h[(258.0±38.3)pg/m L vs.(488.0±56.7)pg/m L]以及36 h[(186.0±43.6)pg/m L vs.(356.0±47.1)pg/m L]BNP水平升高,有统计学差异(P均<0.05)。结论非体外循环较体外循环CABG对心功能的影响较小。 Objective To investigate the influences of cardiopulmonary bypass-coronary artery bypass grafting (CPB-CABG) and off-pump coronary artery bypass grafting (OPCABG) on heart function.Methods The patients (n=40, female 14 and male 26) were chosen from June 2013 to Oct. 2014, and randomly divided into CPB-CABG group and OPCABG group (eachn=20). The plasma samples were collected for detecting level of brain natriuretic peptide (BNP) 1 d before surgery and 12 h, 24 h and 36 h after surgery.Results The level of plasma BNP increased significantly in 2 groups 12 h, 24 h and 36 h after surgery (allP〈0.05). The level of BNP increased after 12 h [(138.0±42.6) pg/mLvs. (266.0±52.1) pg/mL], 24 h [(258.0±38.3) pg/mLvs. (488.0±56.7) pg/mL] and 36 h [(186.0±43.6) pg/mLvs. ( 356.0±47.1) pg/mL] in CPB-CABG group compared with OPCABG group (all P〈0.05).Conclusion OPCABG has less effect on heart function than CPB-CABG.
