摘要
目的检测系统性红斑狼疮(systemic lupus erythematosus,SLE)患者外周血单个核细胞(peripheral blood mononuclearcell,PBMC)微小RNA-206(microRNA-206,miRNA-206)表达水平,探讨其在SLE发病机制中的作用。方法选择2012年12月至2013年11月在沭阳县人民医院确诊的SLE患者,SLE活动性判断采用SLE疾病活动指数(systemic lupus erythematosus diease activity index,SLEDAI);将所纳入SLE患者分为非活动组(SLEDAI≤9分)和活动组(SLEDAI>9分)。采集SLE患者及健康对照者外周血标本,分离PBMC。用实时荧光定量聚合酶链反应检测PBMC中miRNA-206及锌指样转录因子4(Kruppel-like factor 4,KLF4)和孤儿核受体γt(orphan nuclear receptorγt,RORγt)mRNA相对表达量。统计学处理采用t检验和Spearman相关分析。结果共纳入27例SLE患者(非活动组12例,活动组15例)和20名健康对照者。SLE患者PBMC中miRNA-206表达水平显著低于健康对照组,差异有统计学意义[(0.066±0.021)vs.(0.143±0.059),P<0.01];SLE活动组(0.034±0.015)显著低于非活动组(0.125±0.038)与健康对照组,差异均有统计学意义(均P<0.01)。SLE患者KLF4和RORγt mRNA表达水平显著高于健康对照组[(0.637±0.186)vs.(0.104±0.028),P<0.01;(0.575±0.263)vs.(0.065±0.014),P<0.01]。SLE活动组KLF4 mRNA(0.973±0.231)显著高于非活动组(0.131±0.056)与健康对照组,且RORγt mRNA(0.968±0.316)显著高于非活动组(0.094±0.019)与健康对照组,差异均有统计学意义(均P<0.01)。直线相关分析显示,miRNA-206与SLEDAI呈负相关(r=-0.654,P<0.01),KLF4 mRNA与SLEDAI呈正相关(r=0.673,P<0.01),RORγt mRNA与SLEDAI呈正相关(r=0.719,P<0.01);SLE患者miRNA-206与KLF4、RORγt mRNA相对表达量均呈负相关(r=-0.627,P<0.01;r=-0.853,P<0.01),KLF4与RORγt mRNA呈正相关(r=0.684,P<0.01)。结论:SLE患者PBMC中miRNA-206表达水平下降,KLF4和RORγt mRNA表达水平升高,并与疾病活动性密切相关,提示miRNA-206可能在SLE发生发展中起重要作用。
Objective To investigate the expression level of microRNA-206 (miRNA-206)in peripheral blood mononuclear cells (PBMCs)of patients with systemic lupus erythematosus (SLE)and exploe the role of miRNA-206 in the pathogenesis of SLE.Methods Patients with SLE who were diagnosed in Shuyang Hospital from December 2012 to November 2013 were included.The disease activity of SLE was measured&amp;nbsp;by SLEDAI.Patients were divided into two groups:patients with stable SLE (SLEDAI≤9)and patients with active SLE (SLEDAI 〉9 ).Peripheral blood samples were obtained from all patients and healthy controls.PBMCs were isolated by standard density-gradient centrifugation over Ficoll-Hypaque solution.The expression of miRNA-206 and Kruppel-like factor 4 (KLF4)as well as the orphan nuclear receptor γt (RORγt)mRNA was detected by real-time quantitative PCR method.Spearman’s correlation and t-test were used for statistica1 analysis.Results There were totally 27 patients with SLE in the study including 12 patients with stable SLE group and 15 patients with active SLE group.Meanuhile,20 healthy controls were enrolled.The expression level of miRNA-206 in the PBMCs of SLE patients (0.066 ±0.021 )was significantly decreased compared with that of the healthy controls (0.143 ±0.059,P 〈0.01)and the level of miRNA-206 in active SLE(0.034 ±0.015)was lower than that in stable SLE patients (0.125 ±0.038) and norma1 controls (P 〈0.01 ).The levels of KLF4 and RORγt mRNA in the PBMCs of SLE patients were increased significantly than those of the healthy controls [(0.637 ±0.186)vs.(0.104 ±0.028), P 〈0.01;(0.575 ±0.263)vs.(0.065 ±0.014),P 〈0.01].The levels of KLF4 (0.973 ±0.231)and RORγt mRNA (0.968 ±0.316)in active SLE was higher than those in stable SLE patients (0.131 ±0.056 and 0.094 ±0.019)and those in norma1 controls (P 〈0.01 ).The expression level of miRNA-206 was negatively correlated to disease activity (r =-0.654,P 〈0.01).However,the level of KLF4 (r =0.673, P 〈0.01)and that of RORγt mRNA(r =0.719,P 〈0.01 )was positively correlated to disease activity. Furthermore,there was a negative correlation between the expression of miRNA-206 and KLF4 (r =-0.627,P 〈0.01)as well as RORγt mRNA (r =-0.853,P 〈0.01)and the level of KLF4 had positive correlation with that of RORγt (r =0.684,P 〈0.01)in SLE patients.Conclusions The expression levels of miRNA-206 and KLF4 as well as RORγt mRNA in peripheral blood mononuclear cells in SLE patients are abnormal and they may contribute to the pathogenesis of SLE.
