过渡期护理干预对慢性阻塞性肺疾病稳定期患者肺功能及自护能力的影响

THE IMPACT OF TRANSITION PERIOD NURSING INTERVENTION IN COPD LUNG FUNCTION AND SELF-CARE ABILITY IN PATIENTS WITH STABLE

目的探讨过渡期护理干预对慢性阻塞性肺疾病(COPD)稳定期患者肺功能及自护能力的影响。方法将84例稳定期COPD患者随机分为观察组及对照组各42例,对照组实施常规性护理,观察组实施过渡期护理,二组患者分别于入院时及出院8周后应用自护能力测定量表(ESCA)对二组患者自护能力进行评估,并测定二组患者肺功能。结果观察组急性发作次数、再次入院次数显著少于对照组,而发作时间间隔,观察组长于对照组,差异有统计学意义(P<0.05)。观察组干预后自我概念、自护责任感、自护技能、健康知识水平及自护能力评分显著高于对照组,差异有统计学意义(P<0.05)。观察组患儿干预后第一秒呼吸气体的容积(FEV1)、用力呼吸的肺活量(FVC)及FVC占预计值百分比(FVC%)改善优于对照组(P<0.05)。结论过渡期护理干预能有效提高COPD稳定期患者自护能力,改善患者肺功能,减少患者再入院次数,有利于患者预后。

Objective To investigate the effect of the transition of nursing intervention on chronic obstructive pulmonary disease（COPD）patients with stable lung function and self-care capabilities.Methods A total of 84 cases of stable COPD patients were randomly divided into observation group and control group of42 cases,the implementation of routine care control group,the observation group to implement Transitional Care,two groups of patients at the time of admission and discharge,respectively,eight weeks after the application of self-protection capacity measurement scale（ESCA）of the two groups of patients to assess the ability of self-care,and lung function was measured two groups of patients.Results The number of episodes of acute observation group,the number of re-admissions,onset time interval was significantly less than the control group,the difference was statistically significant（P〈0.05）.After observing the intervention group self-concept,self-care responsibility,self-care skills,health knowledge and selfcare ability scores were significantly higher,the difference was statistically significant（P〈0.05）.1s breathing gas volume after the first group of children observed intervention（FEV1）,forced expiratory vital capacity（FVC）and FVC percent predicted（FVC%）improvement than the control group（P〈0.05）.Conclusion The transition of nursing intervention can effectively improve self-care capacity in patients with stable COPD,improve lung function and reduce the number of patient readmission conducive prognosis.
