经髂腹股沟入路内固定治疗C1.1和C1.2型骨盆骨折 被引量:2

摘要 目的:探讨经髂腹股沟入路内固定治疗C1.1、C1.2型骨盆骨折的临床疗效和安全性。方法:2012年6月至2013年8月,收治C1.1、C1.2型骨盆骨折患者17例,男10例,女7例;年龄17-54岁,中位数34岁;车祸伤11例、坠落伤6例;C1.1型6例、C1.2型11例。采用经髂腹股沟入路重建钢板内固定治疗。术后随访观察切口愈合及骨折愈合情况,术后12个月采用Denis标准评定临床疗效,同时观察记录并发症发生情况。结果:所有患者均顺利完成手术,手术时间90-100 min,中位数96 min。17例患者均获随访,随访时间12-25个月,中位数17个月。切口均甲级愈合,未发生重要血管、神经损伤及深静脉血栓形成。术后12个月X线片示所有骨折均愈合;按照Denis标准评定疗效,优13例、良4例。结论:采用经髂腹股沟入路内固定治疗C1.1、C1.2型骨盆骨折,可在仰卧位一次完成骨盆前环和后环骨折的暴露与固定,不会损伤重要的血管、神经,骨折愈合和功能恢复好,值得临床推广应用。
