无支架Baxter主动脉生物瓣的临床应用 被引量:1

摘要 目的总结无支架Baxter主动脉生物瓣临床应用效果。方法以冠状动脉开口下植入方法用无支架Baxter生物瓣作主动脉瓣置换共11例,切除左右冠状窦保留完整的无冠窦,基底缘采用间断缝合,上缘连续缝合。术后采用超声心动图随访12~42个月。结果术后无死亡,1年后2例出现极轻度关闭不全,跨瓣压差均有下降,有效瓣口面积均>1.0cm2,术后早期左室舒缩功能明显改善,心输出量和射血分数明显增加。结论无支架Baxter主动脉生物瓣作主动脉瓣置换效果满意。 Objective To review the clinical effect of aortic valve replacement with Baxter stentless aortic bioprosthesis.Methods The aortic valve was replaced with the Baxter stentless aortic bioprosthesis in11patients.All bioprosthesis valves were implanted using the replacement technique of subcoronary while removing the aortic sinus from the graft and leaving the non-coronary sinus.Using interrupted suture technique on basement and continuous suture on superior border.Echocardiogram was performed to evaluate the function of the bioprosthesis during12-42months postoperatively.Results There was no death after operation.Two patients occurred slight aortic valve incompetence1year after operation.ECHO showed effective valve area(>1.0cm 2 )and good hemodynamic performance.Transvalve gradient pressure decreased.Cardiac output and ejection fraction increased obviously.The systolic and diastolic function of left ventricle were improved obviously.Conclusion The stentless aortic bioprosthesis is an excellent choice for replacement of the aortic valve.
