摘要
目的探讨血清鸢尾素水平与糖尿病肾病(DN)及其临床代谢参数的关系。方法 54例2型糖尿病患者中,合并DN 8例(DN组),肾小球滤过率(GFR)正常者46例(对照组)。检测两组身高、体质量、BMI、血压、骨骼肌量、体脂肪量、体脂百分比、血糖、血脂、肝肾功能指标及血清鸢尾素水平;采用Pearson相关分析法及多元线性回归分析法分析鸢尾素水平与代谢参数的相关性。结果 DN组肌酐水平高于对照组,鸢尾素水平低于对照组(P均<0.05)。鸢尾素水平与Hb A1C(r=0.269,P<0.05)、GFR(r=0.327,P<0.05)、收缩压(β=0.485,P<0.05)、TC(β=1.737,P<0.05)呈正相关关系,与LDL-C呈负相关关系(β=-1.370,P<0.05)。结论 DN患者血清鸢尾素水平降低,其可能参与了DN糖脂代谢紊乱、肾功能损伤的发生。
Objective To investigated the association among serum irisin level, diabetic nephropathy (DN) and it's clinical metabolic parameters. Methods A total of 54 subjects with diagnosed type 2 diabetes were involved in this study, including 8 DN patients (DN group) and 46 normal glomerular filtration rate (GFR) patients (control group). Height , weight, body mass index (BMI) , blood pressure, skeletal muscle, body fat mass, body fat percentage, blood glucose , serum lipid, hepatic function, kidney function and irisin level were measured. The association between serum irisin and metabolic parameters was analyzed by Pearson bivariate correlation and multiple linear regression analysis. Results Compared with control group, the serum creatinine level of DN group increased and the irisin level decreased ( P 〈 0.05). Irisin level was positively correlated with HbA1 C ( r = 0. 269, P 〈 0.05 ), GFR ( r = 0. 327, P 〈 0.05 ) , systolic pressure (f1 = 0. 485, P 〈 0.05 ), total cholesterol(f1 = 1. 737, P 〈 0.05 ). Irisin level was negatively correlated with LDL-C (f1= - 1. 370, P 〈 0.05). Conclusions Serum irisin level was lower in DN patients, which indicated that irisin may be involved in the metabolism disorder of glucose and lipid as well as the kidney impairment of DN.
