人工膝关节置换术中静滴氨甲环酸对围手术期失血量的影响 被引量:9

摘要 目的探讨人工膝关节置换术(TKA)中应用氨甲环酸减少围手术期失血量的效果。方法行TKA的膝关节骨性关节炎患者66例,随机分为观察组和对照组各33例。观察组于术中松止血带前15 min给予氨甲环酸静滴,对照组静滴等量生理盐水。比较两组围手术期失血量、Hb、输血例数、输血量、凝血酶原时间(PT)、活化的部分凝血活酶时间(APTT)、下肢深静脉血栓(DVT)发生率。结果两组术中失血量差异无统计学意义(P>0.05),但观察组术后可见失血量、输血例数及输血量低于对照组,Hb高于对照组(P均<0.05)。两组手术前后PT、APTT比较差异无统计学意义,术后均未发生下肢DVT。结论 TKA术中静滴氨甲环酸能降低患者围手术期失血量,减少术后输血,且不影响凝血功能,不增加下肢DVT发生率。
