广东省城乡地区开展家庭医生式服务的SWOT分析

SWOT analysis carrying out the family doctor service in urban and rural areas of Guangdong

近年,全科团队与家庭医生式服务的相关研究逐步深入,坚持"保基本、强基层、建机制",把握"人人享有基本医疗卫生服务"的医改着力点,在社区卫生服务机构全科团队服务模式的基础上衍生出的"家庭医生式服务"这一更加符合"生物-心理-社会医学"现代医学观念的新型服务模式,以全科医生为责任主体、社区居民及其家庭的健康管理为工作内容、以契约关系为服务形式、提供连续、安全、有效和适宜的综合医疗卫生服务和健康管理的服务模式,这是不断摸索更能贴合居民健康需求服务模式的阶段性创举,充分发挥全科医生团队各角色服务职能的作用,强化个人责任制,增强基层医疗卫生服务能力,构建以社区作为基础、家庭医生式服务作为核心的基本公共卫生体系,为有序、有效地落实开展家庭医生式服务的方案提供针对性的策略建议:增强岗位胜任力,提高家庭医生式服务的居民参与度;增强全科医生(家庭医生)岗位吸引力,订单定向补人力缺口;完善家庭医生首诊、转诊的实现机制等。本研究运用SWOT分析法对"家庭医生式服务"模式存在的优势和劣势,结合当前社会环境的机遇和风险进行系统分析并提出建议,从而充分利用内部优势及规避外部风险,探讨该模式持续发展的可行性。

In recent years,the research related to general practitioners team and the family doctor service advanced step by step. Adhere to the policy of＂guaranteeing of basic,strengthening primary health care,constructing mechanism＂,grasping the medical reform foundation of ＂everyone enjoying basic medical and health services＂,the ＂family doctor service ＂which is more in line with the ＂biological psychological social medicine＂concept of modern medical service model was derived on the basis of the community health service institutions team service mode. This mode takes general practitioner as the responsibility subject,health management for community residents and their families as contents of works,contract as service form,provides continuous,safe,effective and appropriate comprehensive medical and health services and health management. This mode can give full play to each role of general practitioners team in medical service,strengthen the personal responsibility system,improve the health service capacity of primary health care,and build the basic public health system taking community as the foundation and the family doctor service as center. The strategy suggestions for the effective implementation of the family doctor service plan： enhancing competency,improving residents participation in the family doctor service; enhancing the attraction of general practitioner（ GP） position,ordering directional training students can complement manpower gap; improving the mechanism of first consultation and referral by family doctors. SWOT analysis on the advantages and disadvantages of ＂family doctor service＂model were analyzed in this paper,some advice was put forward combining with opportunities and risks under the current social environment. So as to we can make full use of internal strengths and avoid the external risks,explore the feasibility of sustainable development of the model.
