摘要
目的观察1064 nm掺钕钇铝石榴石激光联合胶原蛋白敷贴治疗面部色素沉着的综合临床疗效,为治疗面部色素沉着的患者寻找一种更加安全、便捷、有效的方法。方法将2010年1月—2013年6月在孝感市中心医院门诊就诊的441例面部色素沉着的患者,严格按照纳入标准,随机分为3组,A组148例,B组150例,C组143例;A组用1064 nm掺钕钇铝石榴石激光联合胶原蛋白敷贴治疗,B组单用胶原蛋白敷贴治疗,C组单用1064nm掺钕钇铝石榴石激光治疗;治疗前后要求患者严格防晒、避光。3组分别治疗12周,在治疗0、1、2、4、8、12周分别观察各组色素改变的情况,并记录结果,对结果进行统计学分析。结果 A组总有效例数105例,总有效率70.95%,B组总有效例数49例,总有效率32.66%,C组总有效例数57例,总有效率39.86%;A组与B组、C组比较差异均有统计学意义(P<0.05)。A组未见复发病例及明显不良反应;B组有1例出现不良反应,5例复发;C组出现激光后色素沉着斑7例。结论 1064 nm掺钕钇铝石榴石激光联合胶原蛋白敷贴治疗面部色素沉着疗效确切,治疗效果优于单用胶原蛋白敷贴治疗和单用1064 nm掺钕钇铝石榴石激光治疗,从很大程度上减轻了单用激光带来的不良反应,为临床治疗面部色素沉着提供了新的、有效的治疗方法,可以在临床进行推广。
Objective To observe the clinical efficacy of 1064 nm complex of neodymium doped yttrium aluminum garnet laser therapy combined with collagen dressing in treatment of facial pigmentation,and search a more safe,convenient and effective treatment methods for pigmentation at face. Methods 441 out-patient with facial pigmentation meeting the inclusion criteria in our hospital from January 2010 to June 2013 were randomly divided into three groups,148 cases in group A,150 cases in group B,143 cases in group C. The group A received 1064 nm neodymium doped yttrium aluminum garnet laser combined with collagen dressing treatment,while the group B received collagen dressing treatment,and group C received 1064 nm neodymium doped yttrium aluminum garnet laser treatment. Before and after the treatment,all the patients were away from light or did not expose in the sun. The courses in all three groups were 12 weeks. The changes of pigment were observed on 0,1,2,4,8,12 weeks in the procedures. The results were documented and statistically analyzed. Results The total effective cases in group A were 105 cases with a total effective rate of 70. 95%; the total effective cases in group B were 49 cases with a total effective rate of 32. 66%; and the total effective cases in group C were 57 cases with a total effective rate of 39. 86%; the difference were significant between group A and group B and C( P〈0. 05). In group A,there were no recurrence cases and adverse reactions; in group B,1 case was with adverse reactions,5 cases relapsed after laser therapy; and in the group C,7 cases were with pigmented spots after the laser therapy. Conclusion The 1064 nm neodymium-doped yttrium aluminum garnet laser combined with collagen dressing can achieve a satisfactory effect for the facial pigmentation as compared with each of them alone. It can reduce the negative reactions aroused from single laser therapy,and provide an effective treatment method for the clinical treatment of pigmentation at face.
