摘要 目的:探讨丘脑梗死后认知功能障碍的特点,进一步认识左右丘脑对认知功能的影响。方法:选择年龄、性别、文化程度相匹配的31例首发急胜脑梗死患者和20例健康人进行简易精神状态检查表(MMSE)、蒙特利尔量表(MOCA)的检测,对认识功能进行总体评估。采用《临床医师统计学助手V4.O》统计软件计算。结果:研究组在MMSE、MOCA的检测中记忆力、视空间、执行能力各项目总分及个分项平分上明显低于健康组,差异有统计学意义(t检验,P<0.01):左侧丘脑组在记忆力、视空间、执行能力计算力、注意力测试各分项及总评分上明显比右侧丘脑组低,差异有统计学意义(t检验,P<0.01)。左右丘脑在复述及阅读方面无统计学意义(t检验,P>0.05)。结论:丘脑梗死与认知功能障碍直接相关,不同部位的丘脑梗死所导致的认知障碍特点各有不同,左侧丘脑梗死较右侧丘脑梗死认知障碍重,并且丘脑梗死所导致的认知障碍早期仍以记忆障碍为主多同时伴有视空间和执行能力、注意力及计算等方面不同程度的下降。 Objective: To investigate the characteristics of cognitive dysfunction in thalamic infarction in order to further understand the effects of left and right thalamus on cognitive function. Methods: 31 first- episode patients of acute cerebral infarction with suitable age,sex,culture background and 20 healthy people were tested by mini mental state examination( MMSE),the Montreal scale( MOCA) for the detection of cognitive function and overall evaluation.《Clinicians statistics calculated using statistical software V4. 0 assistant》was used for software calculation. Results: In the detection of MMSE and MOCA,the test scores of memory,visual spatial and executive ability and the total scores are obviously lower than those in the healthy group. The difference was statistically significant( P 〈0. 01). The test scores of memory,visual space,power,calculation ability to execute and attention as well as the total score in left thalamus group are significantly more than those in the right thalamu group,and the difference was statistically significant( P 〈0. 01). The scores on rehearsal and reading in the left and right thalamus had no statistical significance( P 〉0. 05). Conclusion: The thalamic infarction was directly related with cognitive dysfunction. The cognitive impairment features of thalamic infarction in different parts of the causes are different. The infarction cognitive impairment is heavier in left thalamus infarction than in right thalamus. The early cognitive impairment caused by thalamic infarction is featured by memory impairment,accompanied by the declines of visual spatial and executive ability,attention and calculation aspects with different degrees.
