糖尿病人群中各型高血压的分布情况及其影响因素研究 被引量:30

摘要 目的了解糖尿病人群中各型高血压的分布情况,并探讨其影响因素。方法依据开滦研究(Chi CTR-TNC-11001489)的资料,选取符合本研究纳入及排除标准的6 349例糖尿病患者作为研究对象。描述糖尿病人群中各型高血压的分布情况,采用Logistic回归分析探讨糖尿病患者合并各型高血压的影响因素。结果 (1)糖尿病人群中高血压的患病率为51.2%(3 252/6 349),男性为52.6%(2 798/5 318),女性为44.0%(454/1 031)。糖尿病人群中单纯收缩期高血压(ISH)、单纯舒张期高血压(IDH)及双期高血压(SDH)的患病率分别为12.4%(790/6 349)、8.0%(508/6 349)及30.8%(1 954/6 349)。(2)正常血压组和高血压组患者的性别、平均年龄、体力劳动、吸烟、饮酒、喜咸、体育锻炼、收缩压、舒张压、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、体质指数(BMI)、估计肾小球滤过率(e GFR)及超敏C反应蛋白(hs-CRP)比较,差异均有统计学意义(P<0.05)。Logistic回归分析显示,性别、体力劳动、吸烟、饮酒、平均年龄、FPG、TG、LDL、HDL、BMI及e GFR对糖尿病合并高血压的影响有统计学意义(P<0.05)。(3)ISH组、IDH组及SDH组患者间性别、体力劳动、吸烟、体育锻炼、平均年龄、收缩压、舒张压、TC、TG、HDL、BMI、e GFR及hs-CRP比较,差异有统计学意义(P<0.05),饮酒、喜咸、FPG及LDL比较,差异无统计学意义(P≥0.05)。Logistic回归分析显示,平均年龄、FPG、HDL、BMI及e GFR对糖尿病患者合并ISH的影响有统计学意义(P<0.05);性别、饮酒、喜咸、平均年龄、TC、BMI、e GFR及hs-CRP对糖尿病患者合并IDH的影响有统计学意义(P<0.05);性别、体力劳动、吸烟、饮酒、体育锻炼、平均年龄、FPG、TC、HDL、BMI及e GFR对糖尿病患者合并SDH的影响有统计学意义(P<0.05)。结论糖尿病人群中高血压的患病率较高,糖尿病合并不同类型高血压的影响因素存在差异。 Objective To investigate the distribution of various types of hypertension in diabetic population and its influencing factors. Methods From Kailuan research data( Chi CTR- TNC- 11001489),we selected 6 349 eligible cases of diabetes mellitus that conforms to the exclusion standard. The distribution of various types of hypertension in diabetic population was analyzed and logistic regression analysis was undertaken to investigate its influencing factors. Results( 1) The prevalence of hypertension in the diabetic population was 51. 2%( 3 252 /6 349),male 52. 6%( 2 798 /5 318) and female 44. 0%( 454 /1 031). The prevalence of IDH,ISH and SDH was 8. 0%( 508 /6 349),12. 4%( 790 /6 349),30. 8%( 1 954 /6 349)respectively.( 2) There was statistical significance( P〈 0. 05) in the difference between normotension group and hypertension group in terms of gender, average age, physical labor, smoking, alcohol drinking, salt consumption, physical exercise,systolic pressure,diastolic pressure,FPG,TG,TC,LDL,HDL,BMI,e GFR and hs- CRP. The logistic regression analysis showed that the influences of gender,average age,physical labor,smoking,alcohol drinking,FPG,TG,LDL,HDL,BMI and e GFR on diabetes mellitus complicated with hypertension were statistically significant( P〈 0. 05).( 3) The differences among ISH group,IDH group and SDH group in terms of gender,physical labor,smoking,physical exercise,average age,systolic pressure,diastolic pressure,TG, TC, HDL, BMI, e GFR and hs- CRP were statistically significant( P〈 0. 05);the differences among alcohol drinking, salt consumption, FPG and LDL were not statistically significant( P ≥ 0. 05). The logistic regression analysis showed that the influences of average age, FPG, HDL, BMI and e GFR on patients with diabetes mellitus complicated with ISH were statistically significant( P〈 0. 05); the influences of gender, alcoholic drinking, salt consumption,average age, TC, BMI, e GFR and hs- CRP on patients with diabetes mellitus complicated with IDH were statistically significant( P〈 0. 05); the influences of gender, physical labor, smoking, alcoholic consumption, physical exercises,average age, FPG, TC, HDL, BMI and e GFR on patients with diabetes mellitus complicated with SDH were statistically significant( P〈 0. 05). Conclusion There is high prevalence of hypertension in patients with diabetes mellitus. For different types of hypertension in patients with diabetes mellitus,the influencing factors are different.
