耳穴贴压法对血液透析患者生活质量的影响

Influence of Auricular Point Press Therapy on Quality of Life of Patients with Maintenance Hemodialysis

目的探讨耳穴贴压法对血液透析(HD)患者生活质量的影响。方法选取2012年2月—2013年12月河南省中医院血液净化中心符合纳入标准的维持性HD患者78例(均给予常规HD治疗),采用随机数字表法分为治疗组(n=41)和对照组(n=37),治疗组配合耳穴贴压法,对照组配合中药足浴法。治疗15周后采用美国波士顿健康研究所研制开发的生活质量评定简表(SF-36)评价两组患者的生活质量。结果治疗15周后,对照组躯体角色功能(RP)、躯体功能(PF)、躯体疼痛(BP)、生命活力(VT)、情感角色功能(RE)、精神健康(MH)评分较治疗前降低(P<0.05),总体健康感受(GH)和社会功能(SF)评分未降低(P>0.05);治疗组RP、PF、VT、RE评分较治疗前降低(P<0.05),GH、BP、SF、MH评分未降低(P>0.05);且治疗组PF、BP、MH评分高于对照组(P<0.05)。治疗15周后,对照组生理健康(PCS)评分和心理健康(MCS)评分均较治疗前下降(P<0.05),且对照组PCS评分低于治疗组(P<0.05)。结论耳穴贴压法与中药足浴法对改善HD患者生活质量均有一定效果,且耳穴贴压法在改善PF、BP、MH方面较中药足浴法更优。

Objective To discuss clinical effects of auricular point press therapy on life quality of patients with maintenance hemodialysis. Methods 78 cases of maintenance hemodialysis patients（ given conventional treatment, full dialysis） fit into the inclusive criteria by blood purification center of the Henan Province Hospital of TCM,from February 2012 to December 2013,were divided into treatment group（ n = 41） and control group（ n = 37） according to random number table.Treatment group received additional therapy of auricular point press and control group was combined with herbs feet bathing therapy. After 15 weeks＇ treatment,SF- 36 was used to evaluate the life quality of two groups. Results After 15 weeks＇ treatment,evaluation scores of RP,PF,BP,VT,RE,MH in control group had significant decrease（ P 0. 05）,GH and SF scores had no decrease（ P 0. 05）; RP,PF,VT,RE in treatment group had significant decrease（ P 0. 05）,GH,BP,SF,MH score had no decrease（ P 0. 05）. Besides,the evaluation scores of PF,BP and MH in treatment group were higher than that in control group（ P 0. 05）; PCS（ physical component summary） score and MCS（ mentality component summary） score in control group decreased than before（ P 0. 05）, and PCS was lower than that in treatment group with significant difference（ P 0. 05）. Conclusion Auricular point press therapy and herbs feet bathing therapy are both effective in improving the quality of life of maintenance hemodialysis patients. But in improving PF,BP,and MH the former is more superior to the latter.
