摘要 目的探讨冠状动脉慢血流现象(CSFP)的预测因子。方法选择2012年1月—2014年6月于青岛大学附属医院心血管内科行冠状动脉造影术且心外膜冠状动脉狭窄<30%的患者373例,其中明确诊断为CSFP者190例(观察组),血流正常者183例(对照组)。CSFP预测因子的分析采用单因素分析及多因素Logistic回归分析。结果两组患者年龄、收缩压、舒张压、空腹血糖、三酰甘油、总胆固醇、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血小板计数、中性粒细胞计数、嗜酸粒细胞计数、红细胞分布宽度、平均血小板体积、清蛋白、尿酸、肌酐、总胆红素、直接胆红素、间接胆红素、按心率校正后的QT间期(QTc)比较,差异均无统计学意义(P>0.05);观察组患者男性比例、吸烟率、BMI、血细胞比容、尿素氮、碎裂QRS波(f QRS)发生率均高于对照组(P<0.05)。多因素Logistic回归分析结果显示,血细胞比容〔OR=1.069,95%CI(1.017,1.124)〕、f QRS〔OR=14.295,95%CI(6.501,31.437)〕是CSFP的独立预测因子(P<0.05)。结论血细胞比容、f QRS是CSFP的独立预测因子。 Objective To investigate the predictive factors of coronary slow flow phenomenon( CSFP). Methods From January 2012 to June 2014,373 cases undergoing coronary arteriography were selected in the Affiliated Hospital of Qingdao University,and the degree of epicardial coronary artery stenosis of all of them was less than 30%; thereinto,190 cases who diagnosed as CSFP were served as observation group,and the others as control group. Univariate analysis and multivariate Logistic regression analysis were used to analyze the predictive factors of CSFP. Results No statistically significant differences of age,systolic blood pressure,diastolic blood pressure,fasting blood glucose,TG,TC,LDL-C,HDL-C,blood platelet count,neutrophil count,eosinophil count,red blood cell distribution width,mean platelet volume,albumin,uric acid,creatinine,total bilirubin, direct bilirubin, indirect bilirubin and QTc was found between the two groups( P 0. 05); while male proportion,smoking rate,BMI,haematocrit,urea nitrogen and incidence of f QRS of observation group was higher than that of control group,respectively( P 0. 05). Multivariate Logistic regression analysis showed that, haematocrit 〔OR = 1. 069,95% CI( 1. 017,1. 124) 〕 and f QRS 〔OR = 14. 295,95% CI( 6. 501,31. 437) 〕 were predictive factors of CSFP( P 〈0. 05). Conclusion Haematocrit and f QRS are predictive factors of CSFP.
