雾化吸入布地奈德和地塞米松治疗小儿急性喉炎并呼吸困难临床疗效的对比研究

Clinical Effect on Acute Laryngitis Complicated with Dyspnea between Budesonide Aerosol Inhalation and Dexamethasone: A Comparison

目的比较雾化吸入布地奈德和地塞米松治疗小儿急性喉炎并呼吸困难的临床疗效。方法选取2011年3月—2013年7月江西省儿童医院收治的急性喉炎并呼吸困难患儿131例,随机分为观察组66例和对照组65例。对照组雾化吸入布地奈德,对照组静脉滴注地塞米松,比较两组患儿临床症状消失时间、住院时间、治疗24 h后临床疗效及不良反应发生情况。结果观察组患儿吸气性喉鸣消失时间、犬吠样咳消失时间、声音嘶哑消失时间及住院时间均短于对照组(P<0.05);观察组患儿临床疗效优于对照组(P<0.05);治疗期间观察组患儿不良反应发生率为4.5%(3/66),对照组为7.7%(5/65),差异无统计学意义(P>0.05)。结论雾化吸入布地奈德治疗小儿急性喉炎并呼吸困难的临床疗效优于地塞米松,其能迅速缓解患儿临床症状,且安全性高。

Objective To compare the clinical effect on acute laryngitis complicated with dyspnea between budesonide aerosol inhalation and dexamethasone. Methods From March 2011 to July 2013,131 acute laryngitis children complicated with dyspnea were selected in the Jiangxi Provincial Children＇s Hospital,and they were randomly divided into observation group（ n =66） and control group（ n = 65）. Observation group was given budesonide aerosol inhalation,while control group was given dexamethasone intravenous drip. Disappeared time of clinical symptoms,hospital stays,clinical effect after 24 hours of treatment and incidence of adverse reactions were compared between the two groups. Results Disappeared time of laryngeal stridor,bark-like cough and hoarseness,hospital stays of observation group were shorter than those of control group（ P〈0. 05）; the clinical effect of observation group was better than that of control group（ P〈0. 05）. The incidence of adverse reactions of observation group was 4. 5%,that of control group was 7. 7%,the difference was not statistically significant（ P〈0. 05）.Conclusion Budesonide aerosol inhalation has better clinical effect on acute laryngitis complicated with dyspnea than dexamethasone,can quickly relieve the clinical symptoms,is safe.
