采用纤维支气管镜吸痰对呼吸机相关性肺炎患者的治疗效果分析

Analysis on efficacy of aspiration of sputum by bronchoscope for patients with ventilator-associated pneumonia infections

目的探讨纤维支气管镜吸痰处理对呼吸机相关性肺炎(VAP)患者感染的治疗效果以及远期生活质量影响,为临床治疗提供参考依据。方法选取2012年1月-2014年1月VAP患者90例,按数学随机方法分为普通组和治疗组,各45例,普通组实施常规吸痰处理,治疗组实施纤维支气管镜吸痰处理,对比两组患者治疗有效率、临床指标、远期生活质量,采用SPSS 16.0软件进行统计分析。结果治疗组CPIS评分均低于普通组同期评分,对比差异有统计学意义(P<0.05);普通组治疗有效率为62.22%,治疗组有效率为82.22%,两组对比差异有统计学意义(P<0.05);治疗组PaO2为(81.44±0.84)mmHg、二氧化碳分压(PaCO2)为(42.28±0.92)mm Hg、SaO2为(91.88±1.34)%、pH为(7.38±0.06),几项指标改善均优于普通组,对比差异有统计学意义。结论纤维支气管镜吸痰处理对VAP患者感染可以有效控制,对远期生活质量改善明显。

OBJECTIVE To investigate the bronchoscope treatment for aspiration of sputum in ventilator-associated pneumonia（VAP）and the effect on long-term quality of life so as to provide reference for clinical treatment.METHODS Totally 90 VAP patients during Jan.2012-Jan.2014 were selected and were divided into the normal group and the treatment group with 45 cases in each group according to the mathematical random method.The normal group received routine suctioning while the treatment group received aspiration of sputum by bronchoscope for treatment.The two groups of patients were compared for treatment effective rate,clinical indicators,and longterm quality of life.The software SPSS16.0was used for statistical analysis.RESULTS The CPIS score was lower in the treatment group than in the normal group during the same period,the difference was significant（P〈0.05）.The treatment effective rate was 62.22%in the normal group and 82.22%in the treatment group,the difference was significant（P〈0.05）.PaO2was（81.44±0.84）mmHg,carbon dioxide partial pressure（PaCO2）was（42.28±0.92）mmHg,SaO2 was（91.88±1.34）%,PH was（7.38±0.06）in the treatment group,which were all better than the normal group,the difference was significant.CONCLUSION Aspiration of sputum by bronchoscope for treatment of VAP can effectively control infections in patients and significantly improve long-term quality of life.
